■ RECONOZCA los signos de alarma, el continuo monitoreo y reestratificacion de los
casos.
■ Todo paciente febril debe ser interrogado con pensamiento clinico y epidemiologico
(residente o procede de a rea endemica de dengue)
■ precisar el dia que iniciaron los sintomas (primer dia de fiebre)
■ 3 preguntas ba sicas que orientara n a estratificar y a definir el tratamiento a
instaurar en cada el paciente.
■ 1. ¿Tiene dengue?
2. ¿Tiene comorbilidad o signos de alarma?
3. ¿Esta en choque? Tiene alguna otra complicacion en organos?

■ FASE FEBRIL
■ A) El tratamiento en el hogar es:
■ • Reposo
■ • Líquidos orales con SRO en abundante cantidad en pacientes con estabilidad hemodinámica y buena tolerancia de la vía oral.
■ • Acetaminofén en dosis de 10 mg/kg/dosis, cuando temperatura corporal sea de 38 °C ó más, máximo 5 veces en 24 hrs, además utilizar medios físicos, baño con agua al tiempo durante 15 minutos.
■ • No administrar aspirina, ibuprofeno, naproxeno, metamizol, ni diclofenac.
■ • Control clínico y de laboratorio diariamente en la unidad de salud correspondiente al menos durante los primeros 5 días de evolución de la enfermedad.
■ • Plan educativo acerca de los datos de alarma y choque.
■ • No administrar antivirales, ni esteroides.

■ Entre los 3 y 7 di a
■ fuga capilar evidenciado por hemoconcentracion (aumento
del hematocrito), hipoalbuminemia y por la presencia de
derrame pleural, ascitis y edemas generalizados.
■ Dolor abdominal continuo e intenso
n Vo mitos persistentes.
n Derrames serosos en peritoneo, pleura y/o pericardio,
detectados cli nicamente y/o por ima genes.
n Sangrados de mucosas.
■ Somnolencia o irritabilidad.
■ Hepatomegalia mayor de 2 cm.
■ Incremento brusco del hematocrito asociado a un rapidodescenso en el recuento de plaquetas.
¿A QUIEN HOSPITALIZAR?
■ Paciente con 2 rubros del triangulo alterado.
■ ALTERACION NEUROLOGICA
■ Inicie liquidos intravenosos.
■ TECNICA DE HOLLIDAY SEGAR,
■ Restituya el 5% de perdida del volumen circulante.

crystalloid solution and reduce the rate of infusion as mentioned above when the
patient’s condition improves. If the condition is still unstable, repeat the haematocrit after the third bolus.
· Further boluses of fluids may need to be given during the next 24 hours. The rate and volume of each bolus infusion should be titrated to the clinical response. Patients with
severe dengue should be admitted to the high-dependency or intensive care area and be managed by senior staff.
o Clinicians who take care of dengue shock infants should remember that an infant
with a low baseline haematocrit of 30%, presenting with dengue shock and a haematocrit of 40%, is relatively more haemoconcentrated than another child with a
baseline value of 42% and haematocrit of 50% at the time of shock.

Patients with dengue shock should be monitored frequently until the danger period is over.
A detailed fluid balance of all inputs and outputs should be maintained.
Parameters to be monitored include: alertness and comfort levels, vital signs and peripheral
perfusion (every 15-30 minutes until the patient is out of shock then 1-2 hourly). In
general, the higher the fluid infusion rate, the more frequently the patient should be

DX DIFERENCIAL DENGUE
■ Intervention #1: Restrictive resuscitation and “albuminrescue”Iin patients who remained in shock despite 30 ml/kg crystalloid over 3 h (crystalloid refractory shock), colloid wasused for volume replacement, provided hemorrhage hadbeen ruled out.
■ In the ST group, the majority received 4% gelatin(gelofusine) at volumes not >30 m/kg/day.
■ In the ST+ group, albumin was the preferred colloid(“Albumin Rescue”), this was administered at a dose of 1 g/kg as an infusion over 6 h in addition to crystalloids, thetotal volumes of which were titrated to maintain perfusiontargets.
■ Intervention #2: Large volume ascites and intra‐abdominal hypertensionApproach in standard therapy groupIn this group, intra‐abdominal pressure (IAP) monitoring throughbladder pressure measurement was performed in patients whodeveloped tense ascitic collections. If the IAP was elevated, medical treatment followed by percutaneous drainage was initiated
■ Approach in targeted intervention group
■ In this group, rather than waiting for clinically obvious abdominal distension, the IAP was proactively monitored 2 hourly in allpatients who received >30 ml/kg fluid in 3–4 h, and elevated valueswere treated similarly.
■ Intervention #3: Peri‐intubation decompensation and thehigh‐risk intubation management protocolApproach in standard therapy group
■ Peri‐intubation strategies for intubation included lower doses of benzodiazepines and opioids, and in patients who developedperi‐intubation hypotension, fluid resuscitation and inopressorswere initiated. including preference for low‐dose ketamine, preintubation continuous positive airway pressure (CPAP), and inopressor infusions in all cases prior to induction
LABORATORIALES COMPLEMENTARIOS
■ Hematocrito: gui a la terapeutica hi drica, un aumento del 20% por
encima del basal se considera hemoconcentracion
■ Recuento de plaquetas: implicar un aumento del riesgo de sangrado.
■ Leucopenia: muy frecuente, aunque su ausencia no excluye la
enfermedad.
■ los lactantes presentan leucopenia ma s tardi a que otras edades, esto
puede dificul-
■ tar el diagnostico al inicio de la enfermedad y denota la
■ necesidad de pruebas de antigenemia para el diagnostico
■ Hipoalbuminemia: denota la existencia de fuga ca-pilar, esta presente durante la fase cri tica en el dengue grave.
■ Hiponatremia: esta presente con relativa frecuencia en pacientes con dengue.
■ Transaminasas: habitualmente aumentan 2 a 4 veces del valor normal. El aumento superior a 1.000 UI es propio de hepatitis por dengue y es un cuadro grave.
■ Enzimas cardiacas y troponina I cuantitativa ultrasensible: utiles para evaluar una posible miocarditis dentro de un contexto cli nicosugerente.
■ Ecocardiografi a: monitoreo de la funcion ventricular en el paciente grave, deteccion de derrame.
■ Acidosis metabolica: presente en el shock, debe corregirse con volumen oportunamente.
CONCLUSIONES.
■ SOSPECHA CLINICA PARA DENGUE OBLIGA REALIZAR UN
INTERROGATORIO DIRIGIDO Y BASADO EN DIAS.
■ LOS PACIENTES CON CONDICIONES AGREGADAS DEBEN
HOSPITALIZARSE HASTA DESCARTAR SEVERIDAD.
■ ESTADIFIQUE EL PROCESO DE ENFERMEDAD.
■ HIDRATE LENTAMENTE , Y REEVALUE CADA 2HRS
■ EN CASO DE SHOCK POR DENGUE UTILICE COLOIDES MAS EL
PACIENTE DEBE ESTAR CON ADECUADA CARGA HIDRICA.
■ LA ALBUMINA AL 3% ES MEJOR QUE LOS ALMIDONES O
GELATINAS
■ ES UNA ENFERMEDAD SISTEMICA, TRATE LAS
COMORBILIDADES CONFORME SE PRESENTES.USE
