• La infección por VIH es una enfermedad con un acusado componente inflamatorio
• Algunas comorbilidades (enfermedad cardiovascular) se asocian con la persistente inflamación
• Controlar la viremia plasmática no es sinónimo de controlar la inflamación
• Debemos empezar a identificar el TAR como terapia no sólo antirretroviral, sino además antiinflamatoria
• El mecanismo antiinflamatorio principal del TAR es el control estricto de la replicación viral

Consideraciones sobre biterapia
• La doble terapia es experimental
• Su única indicación aprobada por las guías es en el supuesto de evitar toxicidades
• TAF/FTC y ABC/3TC poseen una mínima toxicidad
• La razón de la “simplificación” no debe ser el ahorro a corto plazo
• El presupuesto de farmacia no es un efecto adverso

• Tratamiento de inicio
• Pacientes con supresión viral

– Inflamación persistente
– Comorbilidades de base inflamatoria

• Los ensayos clínicos actuales muestran tasas de respuesta >90%
• Efectos adversos relacionados con el tratamiento <1%

• Monoterapia con IP o con DTG – Descartadas por menor eficacia clínica
• Biterapias con 3TC + segundo agente (DRV/C + 3TC)

experiencia clínica suficiente
• Otras biterapias han fracasado – RAL+ ATV/r

Estrategias de “simplificación”
• ¿Todo esto para quitar ABC o TDF?
– ABC no es tóxico si RCV es bajo
– TAF es mejor que TDF
En estas circunstancias el Dr. Sanz “simplificaría” el tratamiento de sus pacientes
• Pacientes sin tratamiento previo
– Paciente naive con 350.000 copias
– Linfocitos CD4 100/mm3
• Paciente con comorbilidades como:
– Neoplasia anal o displasia grave
– Enfermedad Cardiovascular
– Deterioro cognitivo progresivo
Situaciones en las que no “simplificaría” el tratamiento
• Carga viral detectable
• Nadir de CD4 bajos (<350/μL)
• Inmunodeficiencia (<350 CD4 linfocitos)
• Cociente CD4/CD8 bajo (<0,3)
• Estados inflamatorios persistentes – Obesidad
– Estilos de vida tóxicos (tabaco, alcohol, drogas)
• Enfermedades de patogenia inflamatoria – Enfermedad cardiovascular
– Neoplasias
– Esteatosis/síndrome metabólico
Inmunodeficiencia e Inflamación/activación inmune
• El grado de deterioro inmunológico puede modular la inflamación crónica en pacientes en TAR
