ORACLE publicó que el uso de ATB en RPMp en 4826 mujeres reducía el uso de surfactante pulmonar (RR 0.83, IC=0.72-0.96) y el número de neonatos con US cerebral anormal (RR 0.82, IC=0.68-0.99) . EvidenciasCROWLEY P. CORTICOSTEROIDES ANTES DE UN PARTO
PREMATURO (Cochrane Review). In: The Cochrane Library, Issue 3, 2003. Oxford: Update Software.
Resultados: Se identificaron 18 ensayos que incluían datos sobre más de 3.700 RN de las mujeres aleatorizadas. La administración prenatal de 24 mg de betametasona, 24 mg dexametasona o 2 g de hidrocortisona a mujeres con sospecha de parto prematuro redujo la mortalidad (Odds ratio de 0,60, intervalo de confianza 95%, 0,48 a 0,75), el SDR (Odds ratio 0,53, intervalo de confianza 95%, 0,44 a 0,63) y la hemorragia intraventricular en los neonatos prematuros. Estos beneficios se extienden a una gama amplia de edades gestacionales y no están limitados por factores de sexo o raza. No se han identificado efectos adversos derivados de esta práctica.

Evidencias DOSIS REPETIDAS DE CORTICOSTEROIDES PRENATALES EN
MUJERES EN RIESGO DE PARTO PRETERMINO PARA LA PREVENCION DE ENFERMEDAD RESPIRATORIA NEONATAL
The Cochrane Library, Issue 3, 2003. Oxford: Update Software.Resultados principales: Se reclutaron 550 mujeres en tres ensayos entre las 24 y 30 semanas de gestación. Pocos infantes en el grupo de corticosteroides de dosis repetida tuvieron enfermedad pulmonar severa comparado con los neonatos del grupo placebo (RR 0.64, 95% intervalo de confianza 0.44 a 0.93, 1 ensayo 500 neonatos). No hubo diferencias estadísticamente significativas para cualquiera de los otros resultados que incluyeron otras medidas de morbilidad respiratoria, tamaño PEG al momento del nacimiento, muerte perinatal, hemorragia periventricular, leucomalacia periventricular, y morbilidad infecciosa materna. Pocos neonatos en el grupo de dosis repetidas de corticosteroides recibieron surfactante pulmonar comparado con los neonatos del grupo placebo (RR 0.64, 95% intervalo de confianza 0.44 a 0.93, 2 ensayos, 537 neonatos).

Evidencias Indications for tocolysis are: To inhibit delivery for up to 48 h to allow
the maximal effect of corticosteroids on fetal lung maturity (Goldenberg RL. The management of preterm labor.Obstet Gynecol 2002; 100: 1020–37 & . Slattery MM, Morrison JJ. Preterm delivery. Lancet 2002; 360: 1489–97.
It has been suggested that tocolytic agents should be used only between 24 and 34 weeks of gestation. Rust OA. Preterm delivery: risks versus benefits of intervention. Curr Womens Health Rep 2002; 2: 59–64.

Recomendaciones For women with PROM at term, labor should be induced
at the time of presentation, generally with oxytocin infusion, to reduce the risk chorioamnionitis.
Patients with PROM before 32 weeks of gestation should be cared for expectantly until 33 completed weeks of gestation if no maternal or fetal contraindications exist.
A 48-hour course of intravenous ampicillin and erythromycin followed by 5 days of amoxicillin and erythromycin is recommended during expectant management of preterm PROM remote from term to prolong pregnancy and to reduce infectious and gestational age–dependent neonatal morbidity.

Recomendaciones All women with PROM and a viable fetus, including
those known to be carriers of group B streptococci and those who give birth before carrier status can be delineated, should receive intrapartum chemoprophylaxis to prevent vertical transmission of group B streptococci regardless of earlier treatments.
A single course of antenatal corticosteroids should be administered to women with PROM before 32 weeks of gestation to reduce the risks of RDS, perinatal mortality, and other morbidities.
