SINTOMAS Y SIGNOS
Desde simple Fatiga cuando la enfermedad es leve, disnea debilidad, edema de miembros inferiores, distensión abdominal, palpitaciones, angina, sincope, y en estadios avanzados puede verse cianosis.
Presión art. Normal o disminuida, ingurgitación yugular, reforzamiento del S2 con presencia de S3 o S4. signos de falla derecha (hepatomegalia, ascitis, edemas).

La complicación mas peligrosa es la hipotensión debido a falla VD, y exacerbación de la HTP-
Inodilatadores (Dobutamina, milrrinone), son efectivos en el manejo.
Hipotensión sistémica debida a Falla VD se debe manejar con vasodilatadores como NO o iloprost inhalado.
Inhibidores de 5 fosfodiesterasa (sildenafil, bosentan).

RESUMEN La causa de la HTP debe ser determinada
en ptes perioperatorios y la terapia optimizada para la anestesia.
El punto mas importante en la anestesia es mantener el gasto cardiaco del ventrículo derecho y evitar la hipotensión sistémica.
Vasodilatadores inhalados como NO y prostaciclina pueden ser salvadores en ptes con falla ventricular derecha debida a exacerbación de htp.

CONCLUSION Adecuados avances han permitido el
entendimiento de la fisiopatología de la HTP Desarrollo de Modificadores de la
enfermedad (prostanoides, antagonistas de prostaglandinas e inhibidores de 5 fosfodiesterasa) incrementan calidad de vida y supervivencia.
Manejo anestésico se basa en el entendimiento de la fisiopatología y en evitar falla ventricular derecha e hipotensión sistémica. Current Opinion in Anaesthesiology
