Era un dimecres de finals d’abril, i la

Laura i en Joan eren a classe de medi

amb l’Alícia. Els agradava fer classe amb

ella, sobretot quan se n’anava del tema

del llibre i els explicava tot el que sabia

dels animals. No obstant això, n’hi havia

que a la classe aquell dia no escoltaven

gaire i es dedicaven a altres menesters:

—Ahhh!

—Com pot ser, Sergi! —va exclamar

l’Alícia—. Però es pot saber quina gràcia

hi trobeu, a jugar a enganxar-vos els dits

amb les taules?

It was a Wednesday at the end of April and Laura and Joan were in their Science class with Miss Rius. They liked her classes a lot, especially when they got her off the topic of the book and she started telling them all she knew about animals. But on that day there were some individuals in class who were not listening much and were getting up to all kinds of other tricks:‘Ahhh!’‘Sergi, what is going on?,’ asked Miss Rius. ‘Would you like to explain what is so funny about sticking your fingers between the tables?’

quedar a veure com acabava la història

amb en Sergi perquè havia entrat la

Diana, una mestra mexicana que passava

uns dies a l’escola de visita. Els duia

un encàrrec, i quin encàrrec! Havien

participat en moltes recerques importants

i fascinants, però, fins ara, de la seva

investigació mai no havia depès cap vida

humana de manera tan directa.

Els demanaven a l’Institut d’Investigació

Biomèdica de Bellvitge (IDIBELL) de

l’Hospitalet de Llobregat per col·laborar en

una investigació sobre el trasplantament

de ronyó.

But Laura and Joan could not stay around to find out how things ended up with Sergi, because just then another teacher came into the classroom: it was Diana, a Mexican teacher who had come to visit the school for a few days. She had a job for them. And what a job it was! They had helped out with lots of interesting and important research projects. However, until now, their investigations had never before been responsible for saving human life in such a direct way.The Bellvitge Biomedical Research Institute (IDIBELL) in L’Hospitalet de Llobregat was asking them to help out with a research project on kidney transplants.

—Ai, em fa una mica d’angúnia investigar el ronyó humà.

—A mi també, però segur que quan activem els poders màgics, ens

transformarem en experts de debò en nefrologia i ja no ens farà cosa.

—Llavors, què estem esperant? Dóna’m les mans, Laura. Unim les

energies! —va dir en Joan quatre vegades.

—U-u-unim, les nostres e-e-energiiies! —va repetir la Laura quatre

vegades.

—Ui, ui, ui, Laura, em sembla que amb el singlot que tens no se’ns

activen els poders màgics.

—Co-om que no e-ens havia pa-assat mai.

—Doncs en quin moment que ens agafa l’experiment! —li va fer broma

en Joan, a veure si aconseguia que se li passés—. T’intentaria espantar a

veure si se’t passa, però és que m’agafa el riure.

—Tra-anquil, a-anem tirant i ja se’m pa-pas-sarà.

—Tens raó, segur que de camí a l’Hospitalet ja se t’haurà passat. Anem a

agafar el metro. Tu prova d’anar respirant profundament i vés aguantant la

respiració cada deu segons, a veure si se te’n va.

‘Eww, the idea of investigating the human kidney makes me feel a bit queasy.’‘Me too, but I bet when we activate our magic powers we’ll transform into total experts in nephrology and then we’ll be able to handle anything.’‘Well then, what are we waiting for? Give me your hands, Laura. Combine forces!,’ Joan said four times.‘Co-om-bi-ine, our for-forces!,’ Laura repeated four times.‘Uh-oh, Laura, I don’t think our magic powers are activating with those hiccups of yours.’‘Well, it’s ne-ever happened to us be-efore.’‘Jeez, what a moment to try out the experiment!,’ Joan joked, hoping this might stop the hiccups. ‘I’d try to frighten you to see if that would stop them, but I can’t stop laughing.’‘Don’t wo-worry, let’s get going and they’ll soo-oon sto-op.’‘You’re right. I’m sure they’ll have gone by the time we get to L’Hospitalet. Let’s catch the metro. Try breathing deeply and keep holding your breath every ten seconds. See if that works.’

Fortunately, it didn’t take long for Laura’s hiccups to go away and then they were able to activate their magic powers:‘Combine our forces!’ At last they were able to say it the necessary four times.

‘Uff, I felt quite helpless without the knowledge our magic powers give us.’‘Not to mention the queasiness when you think of those kidneys, eh? Now we can handle anything, can’t we Laura?’‘Now we’re ready to collaborate! Look what it says in this book! No way! It says that one hundred years ago they tried to transplant kidneys from animals to people. Eww, that’s gross!’

‘Don’t be silly, Laura; it’s thanks to those pioneering people that medicine is as advanced as it is today.’ ‘You’re right, Joan. Sorry, I was being silly. That’s what comes from engaging mouth before brain!’

—Laura, no en facis un gra massa, que gràcies a aquella gent pionera la medicina està tan avançada

avui.

—Tens raó, Joan, perdona, ha estat una tonteria, és el que passa per fer comentaris sense pensar.

—Uf, em sentia indefensa sense els coneixements que

ens proporcionen els poders màgics.

—Per no dir l’angúnia de pensar que toques els

ronyons, oi? Ara ja res de res, oi, Laura?

—Ara ja estem a punt per col·laborar! Mira què hi diu,

en aquest llibre, que fort! Com pot ser que fa cent anys

intentessin fer trasplantaments de ronyons d’animals a

persones?

Quan van arribar a la darrera parada de la línia vermella del

metro es van dirigir directament cap a les consultes externes

de l’Hospital Universitari de Bellvitge, ja que allà és on havien

quedat amb una investigadora de l’IDIBELL, que ràpidament

els va acompanyar als laboratoris on estaven investigant

sobre el trasplantament de ronyó.

Allà els van donar la benvinguda quatre persones de l’equip

que estaven participant en la recerca. En David, l’Stephanie,

la Lídia i en Santi, un nefròleg, una cirurgiana, una nefròloga i

un immunòleg. En David i la Lídia els van acabar de posar al

corrent de l’estat de la recerca.

On arrival at the last stop on the red line of the metro they made their way directly to the Outpatients Clinic of the Bellvitge University Hospital, where they had arranged to meet one of IDIBELL’s researchers. He quickly took them to the laboratories where they were carrying out research in kidney transplantation. Once there, they were welcomed by four members of the research team: David, Stephanie, Lídia and Santi; a nephrologist, a surgeon, another nephrologist, and an immunologist. David and Lídia filled them in on the state of their research.

‘We are carrying out research so that people who have kidney transplants won’t have to take so much medication after the operation.’‘Don’t they have to take drugs to stop the body’s defences—the white blood cells—from attacking the new kidney?’‘Exactly, Laura. The main problem we have when we transplant a kidney is that the white blood cells of the person receiving the kidney reject it and attack it.’‘What percentage of rejection are you talking about?’‘We’ve managed to reduce rejection of the transplanted kidney a great deal, and we now have a rejection rate of only twenty per cent, but we still rely far too

much on medicines.’‘It sounds like fantastic news to me that only twenty of every hundred patients reject the new kidney.’‘Yes I agree, it’s not bad at all. But that’s not all; we can now manage to recover eighty per cent of that twenty per cent through the right treatment.’‘That’s fantastic!’

—Estem investigant perquè les persones a les quals fem els trasplantaments de ronyó

no s’hagin de prendre tants medicaments després de l’operació.

—Els fàrmacs se’ls han de prendre perquè els defensors de l’organisme, els glòbuls

blancs, no ataquin el ronyó nou, oi?

—Efectivament, Laura. El problema principal que tenim a l’hora de trasplantar un

ronyó és que els glòbuls blancs de la persona a qui el trasplantem el rebutgin i

l’ataquin.

—I quin percentatge de rebuig teniu?

—Hem aconseguit

disminuir molt el

rebuig al ronyó

trasplantat, ja que

només tenim un

vint per cent de

rebuig, però encara

depenem massa dels

medicaments.

—Jo trobo que està

súper bé que de cent

pacients només el

rebutgin vint.

—Hi estic d’acord,

no està gens

malament. Però

això no és tot, perquè

d’aquest vint per cent ja tenim

capacitat per recuperar-ne, amb tractament,

el vuitanta per cent.

—Que bé!

—Estem investigant perquè les persones a les quals fem els trasplantaments de

ronyó no s’hagin de prendre tants medicaments després de l’operació.

—Els fàrmacs se’ls han de prendre perquè els defensors de l’organisme, els glòbuls

blancs, no ataquin el ronyó nou, oi?

—Efectivament, Laura. El problema principal que tenim a l’hora de trasplantar un

ronyó és que els glòbuls blancs de la persona a qui el trasplantem el rebutgin i ‘Now we’re looking for natural immunosuppressants. In other words, we’re looking into ways of activating the mechanisms that our bodies already have to stop the white blood cells from seeing the transplanted kidney as an enemy that has entered uninvited and that they have to attack to defend the body.’‘It’s like teaching the white blood cells a lesson so that they don’t get it wrong and they don’t go attacking the good guy, right?’‘More or less, Joan,’ answered David laughing, who had found his example very amusing.‘We’re in luck after all! When Diana gave us our mission I thought we were going to have to save a girl or a boy who needed us to finish the research quickly to be able to carry on living,’ Joan whispered to Laura.‘Me too! When you experience something personally, it really feels like it’s much more important and serious than when it happens to others. Do you know what I mean?’‘I know exactly what you mean.’

—Ara estem buscant immunodepressors naturals, és a dir, investiguem la manera

d’activar mecanismes que ja té el nostre cos per fer que els glòbuls blancs no vegin

el ronyó trasplantat com un enemic que entra al cos i l’ataquin perquè es pensen que

defensen l’organisme.

—És com fer que els glòbuls blancs aprenguin la lliçó perquè no s’equivoquin i no

ataquin el bo de la pel·lícula, no?

—Més o menys, Joan —li va respondre rient en David, a qui l’exemple li havia fet

molta gràcia.

—Encara sort!

Quan ens ha passat l’encàrrec la Diana, em pensava que havíem de salvar un nen o

una nena que necessitava urgentment que acabéssim la recerca per poder continuar

vivint —va comentar en veu baixa en Joan

a la Laura.

—Jo també! Realment sembla que, quan

una situació difícil et toca de prop, és molt

més important i greu que les de les altres

persones, oi?

—Ja ho pots ben dir.

But there was no time to waste chatting; they had to make the most of their forty-six hours of magic powers. The research entailed many more risks than they could imagine at that point in time… First of all, they went to the laboratory to analyse the blood samples of two patients to see if they were compatible; in other words, to see if one of them met the ideal characteristics to receive the other’s kidney and to make sure that their white blood cells would not attack the new

kidney as if it was some weird thing invading the body. They had to be quick about it because the kidney had been donated the day before by the family of a girl who had died in an accident.‘Who would have thought it, eh? That new avenues of research would open up as a direct result of the first failures with kidney transplants,’ Joan said.‘Absolutely! We don’t talk enough about the good things that come out of failures,’ David asserted as Laura turned bright red.‘What’s up, Laura? Are you feeling dizzy?’‘No, I think she’s just a bit warm,’ Joan answered, barely concealing his laughter in the face of the strange looks of the rest of the team. ‘I don’t think Laura remembers the story… Can you explain it to her?’‘Good heavens, Joan, she must know it! With her magic powers she’s an expert researcher in kidney transplants.’

Però no tenien gaire temps per a anècdotes, perquè havien d’aprofitar al màxim les

quaranta-sis hores que tenien de poders màgics. La recerca contenia més riscos dels

que es pensaven en aquell moment...

En primer lloc, van anar al laboratori a analitzar unes mostres de sang de dos pacients

per estudiar si eren compatibles, és a dir, si una tenia les característiques idònies perquè

pogués rebre el ronyó de l’altra i els seus glòbuls blancs no ataquessin el nou ronyó

com si fos un estrany que envaeix el cos. No podien trigar gaire perquè el ronyó l’havien

donat el dia anterior els familiars d’una noia que havia mort en un accident.

—Qui ens ho havia de dir, eh?, que arran dels primers fracassos amb els

trasplantaments de ronyó s’obririen noves vies d’investigació —va dir en Joan.

—I tant! No gaires vegades es valoren els encerts que hi ha darrere e

ls fracassos —va comentar en David mentre la Laura es tornava ben vermella.

—Què et passa, Laura? T’has marejat?

—No, segur que només té una mica de calor —els va respondre en Joan,

sense dissimular gaire el riure davant la mirada estranya de la resta de

l’equip—. La Laura em sembla que no recorda la història..., la hi

podeu explicar?

—Home, Joan, segur que ho sap,

amb els poders màgics

és una investigadora

experta

en trasplantaments

de ronyó.

‘Yes, but as you well know, we human beings sometimes forget things very quickly,’ Joan replied, as Laura’s face turned different shades of purple with the sheer embarrassment she was going through.

‘You’ll pay for this,’ she murmured under her breath so the others wouldn’t hear her.‘What’s that, Laura?’‘You’ll pay for this,’ she murmured under her breath so the others wouldn’t hear her.‘What’s that, Laura?’‘Nothing, nothing, just that Joan always reads my mind. He knows how much I love to hear about things from the past.’‘Perfect! No problem! I love history too. It would seem that years after the first failed transplants with kidneys taken from animals, a doctor made a fluke discovery: he was trying to transplant kidneys between people when he found that there was a problem with organ rejection, so much so that patients could not survive if the kidney came from another person, even if that person was their own sister.’‘But the researchers did something really important: they tried to learn from their mistakes. They saw that when a kidney was implanted, our immune system, our white blood cells and antibodies, detected it as something alien and tried to attack it. And that was what opened up new avenues of research to try to find immunosuppressants; in other words, to suppress the body’s natural defence mechanisms.’ ‘After, with transplants between people, things improved a lot thanks to lots of research. But we still rely too much on the drugs that transplant patients have to take.’

—Sí, però ja sabeu que els éssers humans a vegades ens oblidem

molt ràpidament de les coses —s’hi va recrear en Joan, mentre

la cara de la Laura, de la vergonya que estava passant, anava

adquirint tonalitats gairebé morades.

—Me les pagaràs —va xiuxiuejar la Laura perquè no la sentissin els

altres.

—Què dius, Laura?

—Res, res, que en Joan sempre m’endevina els desigs, ell sap que

m’encanta que m’expliquin coses del passat.

—Perfecte, cap problema! A mi també m’agrada molt la història.

Resulta que anys després dels primers trasplantaments fracassats

amb ronyons d’animals, un metge va descobrir per casualitat,

quan provava de trasplantar ronyons entre persones, que hi havia

problemes de rebuig de l’òrgan, tants que la gent no sobrevivia

malgrat el ronyó vingués d’una altra persona, encara que fos la

seva germana.

—Però la gent que investigava va fer una cosa molt bona: intentar

aprendre dels errors. Van veure que el nostre sistema immunitari,

els glòbuls blancs i els anticossos, quan s’implantava un ronyó el

detectaven com un cos aliè i l’atacaven. I això va ser el que va obrir

noves línies de recerca per trobar immunodepressors, és a dir, per

adormir els mecanismes de defensa naturals del cos.

—Després, amb trasplantaments entre persones, això s’ha anat

millorant gràcies a molta investigació, però encara depenem massa

dels fàrmacs que han de prendre.

In the afternoon, to their surprise, the two kids found out that their research was not quite as straightforward as they’d thought: the whole team met up to plan and discuss the best way to carry out what they had to do the following day— transplant a kidney. A wave of emotion came over Laura and Joan: on the one hand they were very excited, but on the other hand they were also a bit scared considering the enormous responsibility of it all, despite the magic powers bestowed on them.Once everything had been decided they went to visit the person chosen to receive the kidney: a boy of fifteen. What a shock that was! Joan and Laura wanted to know what had happened.‘We don’t know for sure, but his kidney is failing and it seems that it can’t clean his blood properly.’

A la tarda, els dos infants van rebre amb sorpresa que la seva recerca no era tan

tranquil·la com s’havien pensat: tot l’equip es va reunir per planificar i discutir la millor

manera de dur a terme el que anaven a fer l’endemà al matí: trasplantar un ronyó. La

Laura i en Joan estaven plens d’emoció, ja que, per una banda, els feia molta il·lusió,

però, per l’altra, malgrat els poders màgics, també tenien una mica de por davant de

tanta responsabilitat.

Un cop va estar tot decidit, van passar a visitar la persona a la qual s’havia decidit que

trasplantarien el ronyó: un nen de quinze anys. Quin esglai! En Joan i la Laura van voler

saber què li havia passat.

—No sabem del cert per què li ha passat, però el seu ronyó s’està fent malbé i ja no li

pot netejar la sang.

—Ara mateix deu ser a

diàlisi. Passarem a xerrar una

estona amb ell i de passada li

explicarem com anirà tot.

—La veritat és que la diàlisi és

un gran avenç, però és molt dur

haver de venir a l’hospital cada dos

dies i connectar-te a aquest ronyó

artificial perquè et netegi la sang.

—I aquí a Catalunya encara

tenim prou sort, que els temps

d’espera per aconseguir un

ronyó oscil·la entre dos i

tres anys.

‘Hello Èric! How are you? We’ve come to tell you how the transplant will go tomorrow.’‘Remember that after the operation you’ll have to spend five days in one of the isolation rooms.’‘You’ve got to be patient, Èric, because we can’t risk you getting an infection in the first few days, as we told you before.’‘Don’t worry, Èric, everything will be just fine!’In the afternoon, Joan and Laura didn’t rest for a second. They were trying to discover a new immunosuppressant thanks to an idea that the transplant research team had been able to further a long way after experimenting on animals. Just then, they were told to leave the laboratory because they had to go and rest before tomorrow’s operation…‘Eureka!,’ a deafening cry was heard by everyone in the vicinity.‘Oh my word, Laura. Can’t you see that we’re bothering people by making noises like that?’‘I’m sorry, I’m really sorry, but I think I’ve got it: we’ve got a new immunosuppressant ready to be tried out tomorrow right after the transplant.’

—Hola, Èric! Com estàs? T’hem vingut a explicar

com anirà demà el trasplantament.

—Recorda que després de l’operació hauràs de

passar-te cinc dies a les habitacions aïllades.

—Has de tenir paciència, noi, però, com et

vam dir, no ens podem arriscar que agafis

una infecció els primers dies.

—Tranquil, Èric, segur que tot anirà bé!

A la tarda, en Joan i la Laura no van

descansar ni un minut, intentaven descobrir

un nou immunodepressor gràcies a una idea

que l’equip d’investigació en trasplantaments

tenia molt avançada després d’experimentar-

la amb animals. Quan ja els estaven fent

fora del laboratori perquè havien d’anar a

descansar per a l’operació de l’endemà...

—Eureka! —un crit eixordador es va deixar

sentir per tota la zona.

—Ostres, Laura! No veus que amb aquests

crits molestem la gent...?

—Ho sento, ho sento, però és que em sembla que

ja ho tinc, tenim un nou immunodepres-sor a punt

per ser provat demà després del trasplantament.

It goes without saying that Laura and Joan were too nervous to get much sleep that night. Early in the morning the entire research team, along with the support of the rest of the medical staff (anaesthetist, urologist, immunologist and nurses), got ready for the transplant. A great team at the service of a great deed: the transplantation of a kidney. The operation was a success, but now it was time to study the evolution of the transplanted kidney, since, in this case, the dosage of the drugs usually administered had been reduced. They had activated the immunosuppressant discovered by Laura and Joan. This was a natural mechanism of the body itself which stopped the white blood cells from thinking that the kidney was a foreign body they had to attack. In order to do this, Laura and Joan had ‘told’ the white blood cells that the kidney belonged to the body by entering the information into the immune system.Everything was going smoothly, but they needed to carry out a constant analysis to make sure that things evolved well and that there were no signs of rejection. Otherwise, the drugs that would neutralise the white blood cell attack would have to be administered immediately.‘Our body is a truly incredible thing.’‘Yes indeed. When everything works well it’s a wonderful machine.’‘It really is like magic. The arteries carry the blood to the kidney and, in turn, the kidney cleans the blood…’‘You’re right, from now on I’ll look at my pee in a different light,’ joked Joan.

Naturalment, aquella nit la Laura i en Joan gairebé no van poder dormir

dels nervis. Ben d’hora al matí tot l’equip d’investigació, amb el suport de

la resta de personal sanitari (anestesista, uròleg, immunòleg, infermeres),

es preparava per al trasplantament. Un gran equip al servei d’una gran

gesta: el trasplantament d’un ronyó.

L’operació va ser un èxit, però ara restava l’estudi de l’evolució del

ronyó trasplantat, ja que en aquella recerca es disminuïen les dosis

de fàrmacs que es donaven habitualment en aquells casos. Havien

activat l’immunodepressor que havien descobert la Laura i en Joan, un

mecanisme natural del cos que feia que els glòbuls blancs no pensessin

que el ronyó era un cos estrany i que l’havia d’atacar. Per aconseguir-

ho, «havien dit» als glòbuls blancs que el ronyó era del seu propi cos

introduint la informació dins el sistema immunitari.

Tot anava com la seda, però calia anar fent anàlisis contínuament per

comprovar que l’evolució era bona i no hi havia

rebuig. En cas contrari, immediatament se li

administrarien els fàrmacs

que neutralitzarien l’atac dels glòbuls

blancs.

—El nostre cos és ben increïble.

—Sí, quan tot funciona bé és una

màquina meravellosa.

—Ho trobo com màgic. Les artèries

porten la sang al ronyó i aquest la

neteja...

—Tens raó, ara em miraré el pipí amb més

respecte —va bromejar en Joan.

‘Well, we’re really sorry but our magic powers are running out and we can’t help you any longer.’‘At least not for a few years.’ ‘Wouldn’t you like to dedicate your efforts to kidney transplant research when you’re older?’‘Definitely! It really is exciting to study how we can make transplants even better. We’ve got two classmates, Lluc and Abril, who will almost certainly want to study it because they are totally into the human body.’‘You’ll have to introduce them to me. Now let’s go and have a good game of paddle tennis to de-stress and then we’ll go for a hearty lunch.’‘Great. You can’t leave on an empty stomach after all the help you’ve given us. Thank
