DEFINICIONES
DEFINICIONES:- DETERIORO DE FUNCIÓN RENAL: incremento de Crs >25% respecto a su basal
- REMISIÓN COMPLETA: proteinuria <0.3 g/día confirmado en dos medicionesseparadas al menos con 1 semana + función renal estable
- REMISIÓN PARCIAL: proteinuria <3.5 g/día o descenso de la proteinuria almenos el 50% en pacientes sin proteinuria nefrótica al diagnóstico + funciónrenal estable.
- RECAÍDA: reaparición de proteinuria nefrótica (>3.5 g/día) en dos en dosmediciones separadas al menos con 1 semana tras haber alcanzado la remisiónparcial o completa.

CONCLUSIONES
● GNMi es una enfermedad autoinmune con ac antiPLA2R positivos en el 70% de lospacientes● Ac antiPLA2Rà ¿Diagnóstico sin necesidad de biopsia?● HISTORIA NATURAL: más del 30% de los pacientes desarrollan remisión espontánea
● Cuándo y si hay que iniciar tratamiento IS está aún por definir
● Tratamiento con ALQUILANTES ha sido probado y es la única terapia confirmada enestudios controlados con “hard renal outcomes»● Tratamiento con Ciclofosfamida tiene importantes efectos secundarios y puede que elseguimiento con antiPLA2R puede disminuir el tiempo de tratamiento
● CNI son eficaces para inducir remisiones pero tiene alta tasa de recaídas
● El tratamiento con ACTH no parece haber demostrado eficacia
● Rituximab puede ser útil para evitar recaídas tras ciclo de CNI● ¿Un inicio temprano de Rituximab podría tener más beneficios que un inicio tardío dealquilantes?
