MIOMATOSIS UTERINA• Es un tumor benigno de células
musculares –Leiomioma-.• Es el tumor más común del útero.• 20-50% de mujeres en edad
reproductiva.• Principal causa de histerectomía en el
mundo.

• La incidencia de complicaciones obstétricas con Miomatosis Uterina es de 37%.
• Más del 50% producen alguna sintomatología.
• Rara antes de menarca y posterior a menopausia.

PATOGENESIS• Son tumores benignos compuestos de
fibras musculares lisas.• Cada una de las células dentro del
leiomioma se origina de una sola célula miometrial.(Townsen y cols-1970-).
• Estudios citogenéticos sugieren que la transformación y crecimiento neoplásico de un mioma es el resultado de rearre-glos cromosómicos en células del miometrio.

ETIOLOGIA• Alta concentración de receptores para
estrógenos en el tejido miomatoso que circunda el miometrio durante todo el ciclo menstrual.

• El metabolismo de los estrógenos dentro de los miomas esta alterado, (estradiol (E2) es pobremente convertido a estrona).

ETIOLOGIA
• El medio ambiente del leiomioma es relativamente estrogénico a pesar de que niveles hormonales sistémicos son normales en la mujer con mioma uterino.

• SUBSEROSO.Pueden ser pedunculados: torsión.
• INTRAMURAL. Es lo más común.• SUBMUCOSO. Comprime el endometrio subyacente.
Produce sangrado.

MIOMA SUBMUCOSO
• Es aquel que distorsiona la cavidad uterina.• CLASIFICACION: Tipo 0 – Leiomioma pediculado sin
extensión intramural. Tipo 1 – Sésil con extensión intramural del
leiomioma < 50% Tipo II– sésil con extensión intramural del

SINTOMATOLOGIA
- Dolor pélvico.- Presión. - Masa pélvica.- Sangrado.- Pérdidas recurrentes de
embarazos.- Infertilidad.

MIOMATOSIS UTERINA
Los leiomiomas, se asocian con un 5% a 10% de casos de infertilidad1, pero los leiomiomas como factor único solamente están presentes en un 2% a 3% de los casos de infertilidad.2

CAUSAS DE INFERTILIDAD
1. Distorsión de la cavidad endometrial.2. Aumento de la distancia en el viaje de esperma para alcanzar las trompas uterinas.3. Obstrucción del ostio tubárico4. Alteración en transporte de los gametos.

CAUSAS DE INFERTILIDAD
5. El aumento del tamaño y contorno del útero puede alterar la implantación.6. Alteración en perfusión sanguínea al endometrio, afectando así la nidación.7. Interferencia con la función de la unión utero-tubarica, comprometiendo la irrigación, o causando irritabilidad miometrial.

• Abdomen: - Masa pélvica irregular
• Vagina: - Sangrado. - Masa transcervical.
• Útero: - Tamaño aumentado. - Irregular.
• Anexos: - Masa anexial (?)

ULTRASONOGRAFIA
• Es el estudio ideal para el diagnóstico de un leiomioma, además permite determinar: tamaño y numero.
• No es invasivo.• Fácil de realizar.• Económico.

ULTRASONOGRAFIACRITERIOS DIAGNOSTICOS:• Masa esférica.• Distorsión del contorno miometrial (por la
masa).• Diferenciación de la masa con el tejido
anexo.• Ecogenicidad del miometrio de tipo
heterogéneo.• En embarazo: Masa > 3 cms.

• Permite valorar defectos de llenado en cavidad uterina.
• Ubica más específicamente a un mioma.
• Mayor beneficio en caso de pacientes candidatas a Miomectomía.

En general, el 80% de leiomiomas uterinos presentes durante el embarazo permanecen del mismo tamaño, o se vuelven más pequeños durante el curso del embarazo.

A) Es la principal causa de histerectomía en el mundo.
B) Su incidencia es alta en la población general.
C) Es causa de infertilidad.D) El USG es el estudio ideal por su alta
sensibilidad y especificidad.

CONCLUSIONESE) Durante el embarazo es esperado un
crecimiento exponencial de los mismos.F) Durante la menopausia es esperada una
disminución en el tamaño y sintomatología.
G) El tratamiento debe individualizarse considerando la paridad y la repercusión en la salud.
