♂♂ 18 a.18 a. Originario y Residente de Tlamacazapan, Originario y Residente de Tlamacazapan, Guerrero.Guerrero.
AHF:AHF: Interrogados y negados. Interrogados y negados.

PADECIMIENTO ACTUAL
Cuadro de 6 meses de evolución caracterizado por:• Diarrea semilíquida, en número de 4-5 al dia, no
asociada a alimentos, ocasionalmente con tenesmo y sangre.
• Fiebre de predominio vespertino y practicamente diaria. Hasta 42ºC acompañada de escalofrios y diaforesis.

PADECIMIENTO ACTUAL
• Pérdida de Peso de aproximadamente 14 kgs.
• Tratamiento con diversos antibióticos: penicilina, amikacina, TMP-SMX, amoxicilina y metronidazol con respuesta parcial y recurrencia.

• INGRESO AL INSTITUTO: 27 mayo 2007
• SV con Fiebre de hasta 41 ºC.• Peso 33.5 kg (previo 48). IMC = 14.8• EF: caquéctico, borde hepático palpable. Guayaco
positivo.

EGD: Ulcera única de 1 cm en tercio distal de esófago
Colonos: Múltiples úlceras con fibrina en ciego, colon ascendente y colon transverso. No fue posible canular valvula ileocecal x deformidad.

TUBERCULOSIS INTESTINAL
• Incidencia creciente aunque aún una enfermedad poco frecuente
• Diagnóstico Díficil (Cancer, EII, otras infecciones gastrointestinales)
• Ppal sitio de afección: Ileocecal y yeyunoileal (77% de los casos). Esófago = 0.3%
• Solo en 20% Tb pulmonar concomitante.

• Díficil diferenciación– Clinica, endóscopica y radiológicamente similares– Área Geográfica ?
• Hasta el 65% de casos de Crohn fueron diagnósticados como Tb Intestinal.
• Tinción ZN: Sensibilidad variable pero baja en general.
• Cultivos: Tiempo.

Sugiere TB: • Granulomas grandes, confluentes y con caseificación. • Úlceras con bordes con histiocitos
ENDOSCOPIA:• Úlceras aftosas con mucosa circundante normal y/o empedrado
