Infecciones en el transplante
• Dependen de un hospedero susceptible y de un patógeno disponible.
• Suelen ser más frecuentes y graves en los primeros 4 meses del transplante.
• Luego de los 6 meses del transplante los niveles de inmunosupresión descienden a niveles aceptables, incluso la mortalidad declina.

Factores predisponentes del hospedero
Transtorno médico subyacente:Hepatitis B, diabetes,bronquitis crónica, enfermedad vesicular
Falta de inmunidad específica (CMV,VEB,HSV,Toxoplasmosis)Colonización previa: Agtes intrahospitalarios, Cándida, S.aureus.
Infección latente previa: TBC,PCP,CMV, VZV;HSV.Medicaciones previas: Inmunosupresores y ATB

Tipo de transplante ú órgano: el sitio de transplante son los lugares más frecuentes de infección.Por isquemia,
lesión, reacción aloinjertoTraumatismo quirúrgico: Estrés quirúrgico.

Flora endógena: la colonizante de boca,
piel y mucosas del tracto digestivo: Cándida spp., bacterias entéricas, cocos gram positivos
Por infección latente de tejidos: Herpesvirus, Micobacterias,

Fiebre en el transplantado• Temperatura igual o mayor a 38°C.• Si hay foco, aunque no halla fiebre, evaluación.• Anamnesis, exámen físico radiología,
hemocultivos, urocultivo, exámen de las muestras respiratorias, recuentos de leucocitos, hepatograma.
• Si no se llega al diagnóstico :virus: CMV VEB, descartar abcesos en el sitio anatómico de la cirugía, reactivación de infecciones (Toxoplasma, Pneumocystis carinii, Cándida).

Fiebre en el transplantado
• No olvidar : rechazo del injerto, reacciones medicamentosas (sobre todo a antiglobulionas anticélulas T), TVP, isquemia por infarto de órganos, reacciones hemolíticas.
• No todas las infecciones se manifiestan con fiebre: con altas dosis de corticoides: shock.
LMP, PCP, meningitis por Crytpcoccus neoformans: raramente fiebre.

Infecciones en Receptores de TMO
• Consiste en la administración ev. tomada de un donante relacionado, no relacionado ó autólogo a un receptor cuyos sistemas hematopoyéticos han sido ablacionados por quimioterapia, ó quimiorradioterapia combinadas.

TMO
• TMO alogénica: utiliza donante con mismo HLA.
• TMO singénicos: provienen del gemelos idénticos con todos los loci genéticos iguales.
• TMO autóloga: MO del paciente que es tratada, luego se ablaciona su MO con QT y se lo rescata con su propia MO.

Indicaciones del TMO alogénico
• Anemia aplásica grave,Leucemia aguda, Leucemia mielogénica crónica,Linfoma, Talasemia mayor,Mieloma múltiple, transtornos genéticos metabólicos
• TMO autólogo: tumores de órganos sólidos y linfomas.

Morbimortalidad pos TMO
• Toxicidad de regímenes preparatorios citotóxicos,la reacción injerto versus hospedero, las infecciones,la recurrencia del proceso maligno subyacente.
• Para prevenir el rechazo, se administra: ciclosporina, corticoides, metrotexato, eliminación de células T de la médula donante,etc.

Patrón temporal de las infecciones:
• 1er mes del transplante: Predominan la granulocitopenia y lesiones
de las mucosas, por la QT pre TMO. Se reactivan infecciones por herpes.

Patrón temporal de las infecciones:
• 2do y tercer mes del transplante:El injerto prende: Aún deterioro profundo de la inmunidad celular y humoral: anergia cutánea,menor respuesta a infecciones por capsulados, infecciones por CMV, Aspergillus, infecciones por cocos G + (catéteres), infecciones por adenovirus, virus BK, V. Sincitial respiratorio, sepsis por Gram negativos.

Patrón temporal de las infecciones:
• Postransplante tardío: 3 meses ó más: recuperación gradual del sistema inmune: dura 1-2 años, si hay enfermedad crónica injerto contra hospedero, demora: más riesgo de infecciones por VZV, sepsis por capsulados, infecciones por S.aureus.

infecciones por Gram + si hay catéteres ev.• Cándida y Aspergillus son frecuentes en TMO.• Infecciones por herpesvirus: reactivación o infecciones
primarias, afectan cavidad oral, esófago, área genital.• Infecciones por CMV: 50% de los TMO: más aún si tienen
anticuerpos contra CMV pre Transplante.Neumonitis por CMV: mortalidad 80%

Transplantes de órganos Sólidos• Receptores de transplante cardíaco:
inmunosupresión más importante que en los receptores de transplante renal.
• Infecciones pulmonares por gérmenes tanto hospitalarios como extrahospitalarios.
• Mediastinitis: por S.aureus y S.epidermidis. Mycoplasma hominis y Legionella.
• Toxoplasmosis (quistes en miocardio del injerto), infección por VEB.
• No mayor riesgo de endocarditis

Receptores de transplante renal: • Infecciones urinarias: por enterobacterias,
Pseudomonas aeruginosa y Cándida spp.• Infecciones virales: asintomáticas: Papovavirus,
virus BK ó JC, • Mycoplasma hominis.• Reactivación de TBC.• Más riesgo: altas dosis de corticoides, antecentes
de tratamiento por rechazo, CMV y hepatitis C.

• Alta incidencia de infecciones bacterianas pulmonares.
• Mediastinis, infecciones micóticas invasivas neumonía por PCP y CMV.
• Bronquiolitis oblitarente en el pos tranplante tardío.
• Por el acto quirúrgico hay ablación del reflejo tusígeno.
• Rechazo crónico.

Receptores de Transplante hepático
• Más riesgo de infecciones micóticas diseminadas (Cándida spp.)
• El hecho de realizar anastomosis en sitios no estériles: abcesos hepáticos; por estrecheces de la anastomosis realizadas: gérmenes de la flora GI.
• Colangitis: Por la cirugía• Hepatitis por CMV, herpes. A veces tan grave que
requiere nuevo transplante.

Diagnóstico y control
• Conocimiento de la flora hospitalaria.• Estado serológico antes del transplante, tanto del
donante como del receptor.• Cultivos de vigilancia: virales para CMV y HSV.• Medidas profilácticas: TMP-sulfametoxazol
contra Pneumocystis carinii. Aciclovir en aquellos con antecedentes de herpes. Otros: fluconazol, Pirimetamina, decontaminación del tubo digestivo.
