Falla hepática e hipertensión portal
Creatinina mayor de1,5 mg/dl o depuraciòn de

Falta de respuesta a expansión de volúmen
Proteinuria menor de 500 mg/24 horas y ecografía
renal normal

Síndrome Hepatorrenal. Criterios diagnósticos principales
1. Disminución del filtrado glomerular (creatinina sérica > 1,5 mg/dl o Cl
creatinina de 24 h < 40 ml/min)
2. Ausencia de shock, infección bacteriana, pérdida de líquidos o
tratamiento actual con fármacos nefrotóxicos
3. No mejora sustancial de la función renal (disminución creatinina por
debajo de 1,5 mg/dl) tras la suspensión de los diuréticos y la expansión
con 1,5 l de suero fisiológico
5. Proteinuria < 500 mg/día y no evidencia ecográfica de uropatía
obstructiva o nefropatía parenquimatosa

Síndrome Hepatorrenal. Tipos
Tipo I
Insuficiencia renal rápidamente progresiva. Aumento superior o igual al 100% de la creatinina sérica inicial hasta alcanzar un nivel superior a 2,5 mg/dl, o una disminución del Cl de creatinina a un valor < 20 ml/min, en menos de 2 semanas.
Tipo II
Afectación de la función renal (creatinina sérica >1,5 mg/dl) que no cumple los criterios del tipo I.

Tratamiento
1. Evaluar trasplante hepático.
2. Utilizar diuréticos para el tratamiento de la ascitis
sólo si aumentan significativamente la natriuresis
(>30 mEq/día).
3. Tratar la recidiva de la ascitis mediante paracentesis
repetidas con albúmina ev.
4. Restringir el aporte de líquido si existe hiponatremia
