La Ciencia de siempre
Los tiempos de la “nueva normalidad” están constituyendo una época, cuando
menos, nada normal. Algunas de sus características, por todos conocidas y comentadas,
son la incertidumbre, la rigidez, la crítica, y por qué no decirlo, la contradicción.
Contradicción en las normas, en los protocolos, en la información… En las normas que
cambian y se contraponen a lo largo del tiempo, en los protocolos que chocan con una
realidad en la que su desarrollo no siempre es posible, en la información muchas veces
poco clara y, de sentencias y declaraciones de escasa base científica y pobres
evidencias.
Éste es el punto en que queremos incidir. Desde Labmed no pretendemos ser
agitadores, pero sí queremos reivindicar la ciencia; la que es pausada no frenética, la
que es crítica no sensacionalista, la que es reflexiva y no histérica, la realizada con rigor
y no precipitadamente. En pocas palabras, la ciencia de siempre.

Somos conscientes de las limitaciones actuales: tiempos de respuesta necesariamente cortos, contadas
evidencias, recursos limitados, alto nivel de exigencia y presión por las organizaciones científicas, sanitarias y
sociales… Pero si la ciencia se convierte en una “carrera por publicar” en el que lo principal es ser el primero y a
cualquier precio, o en un salvavidas improvisado para justificar ante las instituciones y la sociedad que se está
“haciendo algo”, sea lo que sea; la credibilidad, la seguridad y la autoridad que siempre han sido propias del
trabajo científico quedarán sustituidas por sus antónimos.
En medio de tanta agitación es preciso ser firmes, manteniéndonos leales al método científico; sólo así
podremos hacer frente a una situación tan desconcertante como la actual y ganar esta batalla, para poder decir
con verdadera confianza que:
“Todo saldrá bien”

presenta (analitos y cálculos asociados). Este es un
hecho frecuente que sucede con la bioquímica
urinaria automatizada.
Para facilitar su interpretación, en nuestro
laboratorio se decidió dividir el informe de bioquímica
urinaria en tres apartados atendiendo a la fisiología de
la nefrona en cuanto a los procesos de filtración,
reabsorción o secreción. De esta forma, se consigue
una evaluación funcional estructurada.
En el apartado de la “función glomerular” se
encuentran aquellas sustancias que sufren libre
filtración en el glomérulo y no experimentan ni
reabsorción ni secreción (o bien es despreciable).
Este es el caso de la urea y la creatinina, además del
cálculo del filtrado glomerular estimado (FGe) con la
fórmula CKD-EPI. También se incluyen las proteínas
totales que, en ausencia de patología glomerular,
encontraremos en orina en muy bajas
concentraciones. Por último, existen numerosas
proteínas específicas para evaluar el daño a este nivel
como es la albúmina, que debido a su carga y tamaño
no es filtrada en condiciones normales y sólo la
encontraremos en orina en casos de daño glomerular.
Atendiendo a la “función tubular proximal”, se
informan aquellos analitos cuya tasa de reabsorción
es máxima a nivel tubular como glucosa, ácido úrico y
fosfato. Junto a estos, la 2-microglobulina aporta
información más específica acerca de afectación
tubular puesto que en ausencia de patología es
reabsorbida en su mayor parte en esta fracción de la
nefrona.
Por último, en el bloque dedicado a la “función
distal” se incluyen la mayor parte de electrolitos ya que
es donde fisiológicamente se sitúan numerosos
transportadores activos y es la parte más susceptible
a la acción del sistema endocrino. A pesar de que
muchos de ellos sufren reabsorción a lo largo de toda

lab we decided to divide urinary laboratory reports into
three physiological parts based on the renal location
where filtration, reabsorption or secretion from each
analyte are mainly performed, achieving a complete
functional evaluation.
In paragraph “glomerular function”, we find
those analites that have free glomerular filtration and
negligible reabsortion and secretions rates as in the
case of urea and creatinine. With this last one,
glomerular filtration rate (GFR) using the CKD-EPI
equation is also estimated. Total proteins results are
also included in this section. In absence of glomerular
pathology, their urinary concentrations are expected to
be very low. In addition, there are other specific
proteins used to evaluate kidney impairment at this
level as albumin, which due to its charge and size it is
not usually filtered in normal conditions.
In the section “proximal tubule function”, we
report those tests with the highest reabsorption rate
asglucose, uric acid and phosphate. In conjunction to
these tests, 2-microglobuline give us more
information regarding to tubular impairment due to its
highest reabsorption rate at this fraction in normal
conditions.
Finally, in the paragraph “collecting duct
function”, we include most of the electrolytes because
many of their active transporters are located here and
it is the more susceptible fraction to endocrinological
effects. Despite most of these analytes have
reabsortion processes along the entire nephron, a
good analysis of the electrochemical urinary balance
can be performed given that these analytes are
responsible for urinary osmolality and final
concentration.

la nefrona por distintos canales, podemos realizar un
buen análisis del equilibrio electroquímico ya que son
responsables de la concentración y osmolalidad final
de la orina.
Para concluir, estas agrupaciones permiten
adjuntar comentarios interpretativos a cada apartado
(de manera automática o no) que mejoran la calidad
de la información aportada por el laboratorio y el buen
uso de la misma por parte de los clínicos. Es
recomendable que estos comentarios estén
armonizados siempre con el fin de cumplir los
requisitos de la norma ISO 15189.
In conclusion, these paragraphs could allow us
to make interpretative comments to each section
automatically in order to improve the quality of clinical
laboratory information and the good use of these
reports by clinicians. It is recommended that these
comments are always harmonized to meet the
requirements of ISO 15189.

Se presenta una imagen de una extensión de
sangre periférica teñida con May-Grünwald-Giemsa
(figura 1) en la que se observa la presencia de
linfocitos binucleados. Estas entidades son una
característica constante y casi exclusiva de la
linfocitosis B policlonal persistente (LBPP).

paciente de 38 años fumadora. En el hemograma se

lo que se decidió estudiar al microscopio la extensión
de sangre periférica encontrándose la presencia de
linfocitos binucleados (<10 %), lo que orientó el
diagnóstico hacia una LBPP.
La LBPP es una entidad poco frecuente
descrita por primera vez en 1982 por Gordon et al. que
afecta principalmente, pero no de manera exclusiva, a
mujeres fumadoras jóvenes o de mediana edad,
caracterizada por una linfocitosis leve-moderada
mantenida que suele oscilar entre 5 y 15 x 109/L
aunque existen casos en que la cifra de linfocitos no
está aumentada. Suelen ser pacientes clínicamente
asintomáticos.
Los criterios diagnósticos de LBPP, además de
la morfología linfocitaria característica, son un
aumento policlonal de IgM en suero con valores
normales o bajos del resto de inmunoglobulinas. Los
linfocitos expresan siempre marcadores de linfocitos
B maduros como CD19, CD20, CD22, CD79b, CD27
o FMC7 y la ausencia de marcadores como CD5,
CD10, CD23, CD43 o CD103, así como también una
relación kappa/lambda normal.
Es importante y necesario, ante el hallazgo de
la binuclearidad linfocitaria, realizar el diagnóstico
diferencial con otros síndromes linfoproliferativos
monoclonales malignos como la leucemia
prolinfocítica B, la tricoleucemia o el linfoma de la zona
marginal esplénico, corroborando el origen policlonal
de la entidad mediante inmunofenotipo, citogenética y
biología molecular.
En el presente caso, para confirmar la
orientación diagnóstica, se amplió la determinación de
inmunoglobulinas y el proteinograma en suero, así
como el inmunofenotipo en sangre. Los resultados
obtenidos mostraron un aumento de la IgM
(1031_mg/dL) sin alteraciones en el proteinograma.
Además, el inmunofenotipo también fue compatible
con una LBPP destacando la expresión de

LBPP is a rare entity first described in 1982 by
Gordon et al. which mainly affects, but not exclusively,
young or middle-aged women, usually smokers. It is
characterized by a maintained mild-moderate
lymphocytosis that usually ranges between 5 and
15_x_109/L although there are cases with normal
count of lymphocytes. They are usually clinically
asymptomatic patients.
The diagnostic criteria for LBPP, besides the
characteristic lymphocyte morphology, are a
polyclonal increase of IgM levels with normal or low
values of the rest of immunoglobulins. Lymphocytes
always express markers of mature B lymphocytes
such as CD19, CD20, CD22, CD79b, CD27 or FMC7
and the absence of markers such as CD5, CD10,
CD23, CD43 or CD103, as well as a normal kappa /
lambda ratio.
It is important and necessary, given the find of
lymphocyte binuclearity, to make the differential
diagnosis with other malignant monoclonal
lymphoproliferative syndromes such as B
prolymphocytic leukemia, tricoleukemia or splenic
marginal zone lymphoma, corroborating the polyclonal
origin of the entity by immunophenotype, cytogenetics
and molecular biology.
In the present case, in order to confirm the
diagnostic orientation, the determination of
immunoglobulins and proteinogram in serum was
extended, as well as the immunophenotype in blood.
The results obtained showed an increase in IgM
(1031_mg/dL) without alterations in the proteinogram.
In addition, the immunophenotype was also
compatible with a LBPP highlighting the expression of
mature B lymphocyte markers as well as the absence

marcadores de linfocitos B maduros así como la
ausencia de CD5 y la presencia de cadenas de

Finalmente, cabe mencionar que todavía hoy
falta dilucidar si la LBPP se trata de una patología
benigna o de una condición premaligna. Por esta
razón, se recomienda un seguimiento de dichos
pacientes.

Se presenta el caso de un varón de 49 años,
VIH positivo, que acude al hospital por alteración en la
escritura de forma brusca sin otra sintomatología
asociada. Se le realiza resonancia magnética nuclear
cerebral, mostrando una lesión ocupante de espacio
temporoparietal derecha con afectación cortical con
realce en anillo y edema perilesional asociado. Se le
pauta tratamiento corticoideo oral y el paciente queda
asintomático.
Ante la alta sospecha de patología de origen
infeccioso se realiza diagnóstico diferencial de los
patógenos cerebrales más comunes. Entre los
resultados destaca la serología para Toxoplasma
gondii (IgG positiva, IgM negativa) junto con ADN
negativo en líquido cefalorraquídeo. Se desconocen
resultados serológicos previos porque el paciente
viene de otro hospital. Se retiran corticoides de forma
paulatina y se inicia tratamiento empírico con
pirimetamina y sulfadiazina para probable
toxoplasmosis cerebral.
En el laboratorio de urianálisis se realiza un
estudio básico de orina (sistemático y sedimento
urinario). En el sedimento destaca la presencia de
cristales medicamentosos en todos los campos
examinados (figura 1).
La morfología de los cristales de sulfadiazina
depende de la forma del fármaco que cristalice
(inalterado o metabolizado). La sulfadiazina libre
forma cristales densos y globulares, mientras que su
metabolito hepático, la acetilsulfadiazina, forma
cristales con forma de gavilla de trigo con unión
excéntrica. En ambas orinas del paciente se observan
cristales de acetilsulfadiazina (figuras 1a y 1b).
Independientemente de la forma del
medicamento, éste es eliminado vía renal. Su
solubilidad depende del pH urinario, siendo insoluble
en orina ácida. En estas condiciones se pueden
A 49-year-old male HIV positive patient came to
hospital for sudden impairment of his writing capacity
of new onset without other associated symptoms. A
cerebral nuclear magnetic resonance was performed,
showing a space occupying lesion in the right
temporoparietal space with cortical involvement, ring
enhancement and associated perilesional edema. He
began oral corticosteroid treatment and became
asymptomatic.
Given the personal background of the patient
differential diagnosis was made presuming an
infectious origin. The serology for Toxoplasma gondii
showed positive IgG, negative IgM along with negative
DNA in CSF. Previous serologic results were not
assessed because patient came from another
hospital.
Corticosteroids were removed gradually and
empirical treatment was initiated with pyrimethamine
and sulphadiazine for probable cerebral
toxoplasmosis.
Urine samples were analysed in the laboratory.
Microscopic study of the urine sediment showed the
presence of pharmacologic crystals in all the
microscopic fields examined (figure 1).
The morphology of the sulphadiazine crystals
depends on the form of the drug that crystallizes
(unchanged or metabolized). Free sulphadiazine
forms dense and globular crystals, while its hepatic
metabolite (acetylsulphadiazine) has a wheat sheaf-
shaped with eccentric binding morphology.
Acetylsulphadiazine crystals are observed in both
urine samples of the patient (figures 1a and 1b).
Regardless of the form of the medication, it is
excreted by the kidneys. Its solubility depends on
urinary pH, being insoluble in acidic urine. Under these
conditions the drug can crystalize and cause acute

generar cristales que pueden provocar un fracaso
renal agudo (pH aproximado de 5,5).
Para prevenir la cristalización de la sulfadiazina
es fundamental mantener una hidratación adecuada y
alcalinizar la orina. Además, se recomienda evitar la
hipoalbuminemia porque aumenta la fracción libre del
fármaco, favoreciendo así la cristaluria.

El pH de la primera orina fue de 6 y el paciente
presentó función renal normal. Ante la observación de
abundantes cristales de sulfadiazina se comunicó al
clínico y se consiguió monitorizar al paciente con una
segunda orina dos días después. A diferencia de la
primera orina, la segunda fue de color marrón y gran
turbidez. Además, presentó un sedimento mayor tras

cristales de sulfadiazina alcanzaron mayor tamaño
respecto del sedimento de la primera orina. El pH
urinario se mantuvo en 6 y la función renal inalterada.
A pesar de ello, se decide instaurar tratamiento precoz
con bicarbonato sódico (NaHCO3) como profilaxis
para evitar un episodio de fracaso renal agudo en los
días siguientes.
Sulphadiazine crystals were bigger comparing to the
first urine. Urinary pH and patient renal function was
unchanged. Despite this, patient started early
treatment with intravenous NaHCO3 as prophylaxis to
avoid an episode of acute renal failure in the following
days.

Se presenta una imagen de una extensión de
líquido pleural teñida con panóptico rápido en la que
se pueden observar abundantes células atípicas y
fenómeno de canibalismo celular (figura 1) que
sugieren origen neoplásico. La situación descrita se
observó en multitud de campos de la extensión, junto
con abundantes nidos celulares constituidos por
células atípicas que además presentaban procesos de
mitosis (figura 2).
An image of a pleural fluid smear stained with
quick panoptic is presented. The figure shows
abundant atypical cells and cell cannibalism
phenomenon that suggest a neoplastic origin
(figure_1). The situation described was observed in
many fields of pleural fluid smear, along with abundant
atypical nest cells with mitosis phenomena (figure 2).

El canibalismo celular, también denominado
entosis, ocurre cuando una célula rodea, mata y
digiere a otra. Las causas y mecanismos implicados
en este proceso no se conocen bien, pero sí que se
ha observado su relación con células tumorales,
debido a su capacidad de proliferar desprovistas de la
matriz extracelular y a su alto índice de división y
producción de células mitóticas1.
Las figuras mitóticas observadas en algunos
tipos de líquidos de origen tumoral se producen
durante la división celular regulada por la activación y
Cellular cannibalism, also called entosis, occurs
when a cell surrounds, kills and digests another. The
causes and mechanisms involved in this process are
not well known, but they have been associated with
tumour cells due to their ability to proliferate without
the extracellular matrix and their high rate of division
and production of mitotic cells1. The mitotic figures
observed in some types of fluids of neoplastic origin
are produced during cell division due to the activation
and deactivation of the cell cycle regulatory proteins.
Cell cycle deregulations are closely related to the

desactivación de proteínas del ciclo celular, cuya
pérdida de control está estrechamente relacionada
con la aparición de cáncer2. Las células tumorales
acumulan mutaciones que favorecen su proliferación
descontrolada, y el aumento del número de células
mitóticas que pueden ser visualizadas por
microscopía3 (figura 2). Por tanto, al igual que el
canibalismo, la mitosis suele observarse con
frecuencia en líquidos de origen tumoral (figura 2),
propiedades que han sido sugeridas como
mecanismo de ventaja del tumor para la supervivencia
en condiciones adversas bajas en nutrientes3.
Las imágenes presentadas pertenecen al caso
de una mujer de 62 años con diagnóstico de
carcinoma de ovario estadio IV de dos años y medio
de evolución, tratada con múltiples líneas de
quimioterapia.
Debido la evolución clínica de la paciente, con
múltiples derrames pleurales y ascitis, se realizó una
toracocentesis evacuadora, y se remitió muestra del
líquido pleural al laboratorio de análisis clínicos para
estudio celular y bioquímico. Los resultados analíticos
obtenidos fueron leucocitos 809 cel/μL, proteínas
totales de 4,5 g/dL, LDH en líquido de 779 U/L y LDH
sérica de 485 U/L; que sugerían origen exudativo
debido al valor elevado de proteínas totales (>3 g/dL)
y relación LDH fluido/LDH suero >0,6.
La observación por microscopía mostró
celularidad constituida fundamentalmente por células
no sanguíneas, con intensa basofilia, elevada relación
núcleo-citoplasma, de gran tamaño, agrupadas en
formación de nidos y abundantes mitosis celulares
(figura 2). Debido a los hallazgos encontrados, la
muestra se remitió a anatomía patológica, cuyo
estudio citológico reveló un marcado pleomorfismo
nuclear compatible con carcinoma papilar que
confirmó la sospecha de metástasis a nivel pleural.
Tras el análisis del líquido pleural se decidió analizar
appearance of cancer2. Tumour cells accumulate
mutations that support their uncontrolled proliferation
and increase the number of mitotic cells that can be
visualized by microscopy3 (figure 2). Therefore, like
cannibalism, mitosis is often observed in fluids of
neoplastic origin (figure 2), properties that have been
suggested as a mechanism of tumoral advantage for
survival in low adverse nutrient conditions3.
The images presented belong to the pleural
effusion of a 62-year-old woman. The patient
presented two and a half years of evolution of a stage
IV ovarian carcinoma treated with multiple
chemotherapy lines.
Due to the clinical evolution of the patient with
multiple pleural effusions and ascites, an evacuating
thoracentesis was performed. A pleural fluid sample
was sent to the clinical analysis laboratory for cell and
biochemical study. The obtained analytical results
were leukocytes 809 /μL, total proteins 4,5 g/dL, fluid
LDH 779 U/L and serum LDH 485 U/L, which
suggested exudative origin, due to the elevated level
of total proteins, above 3 g/dL, and the ratio of fluid
LDH/serum LDH >0,6.
The microscopic observation showed a
cellularity constituted mainly by non-blood cells, with
intense basophilia, high nucleus-cytoplasm ratio, big
size cells, grouped in nest formation and abundant
cellular mitosis (figure 2). The sample was sent to the
pathology laboratory to confirm the findings, and the
cytological study revealed a marked nuclear
pleomorphism compatible with papillary carcinoma,
which confirmed the suspicion of pleural metastases.
Ascitic fluid was also analyzed due to the
previous result in pleural fluid with similar findings. The
patient was followed up by the oncology service,
where multiple cycles of chemotherapy were
administered. The patient underwent surgery in which
a hysterectomy, doble bilateral anextomy,

el contenido del líquido ascítico en el cual se
encontraron hallazgos similares.
La paciente ingresó en planta bajo seguimiento
de oncología, donde se le administraron múltiples
ciclos de quimioterapia. Fue sometida a intervención
quirúrgica en la cual se le realizó histerectomía, doble
anextomía bilateral, linfadenectomía (pélvica bilateral
y paraóptica), apendicectomía y omentectomía. Pese
al tratamiento quimioterápico, quirúrgico y de soporte
la paciente presentó una evolución tórpida y
empeoramiento progresivo que resultó en éxitus.

surgical treatment and support, the patient presented
a poor evolution and a progressive worsening resulting
in exitus.
