- A series of five human-to-guinea pig TB transmission studies was conducted to testvarious interventions for infection control
- Despite exposure to presumably infectious MDR-TB patients, infection percentagesamong guinea pigs ranged from 1% to 77% in the five experiments conducted.
- In one experiment in which guinea pigs were exposed to 27 MDR-TB patients newlystarted on effective treatment for 3 months, there was minimal transmission.
- In four other experiments with greater transmission, guinea pigs had been exposedto patients with unsuspected extensively drug-resistant tuberculosis who were not oneffective treatment.
- CONCLUSIONS : In this model, effective treatment appears to render MDR-TB patientsrapidly noninfectious.

Historia Natural Infección HIV sin ARV
El TARGA baja la incidencia de TB (reduccion del 65%)a todos los niveles de CD4
El inicio temprano del TARGA es claveen el control de TB/VIH.
En 2013 la OMS ha publicado las NuevasRecommendaciones sobre el tratamiento del
VIH/SIDA:ARVs a todos los que tienen CD4 abajo de
500 cels/mm3….
…pero nuevas recomendacionespodrian llegar en breve….
…pues hay nueva evidencia
816 participantes: grupo ART precoz empiezo cuando CD4: 200-350 células/mm3 (en lasprimeras 2 semanas), y grupo ART diferido empiezo cuando caída de CD4 <200 células/mm3(en una mediana de 1,3 años).Después de 5 años CD4 > elevados en el grupo precoz (496 células / mm3, 95% CL 477-515,frente a 373 células/mm3 P 0,0001):Riesgo de TB mayor en el grupo diferido (HR 2.41, IC 95%: 1,56 a 3,74; P, 0,0001) yfuertemente correlacionado con un menor recuento de CD4.
Conclusion: el retrasar el inicio del TARGA cuando los CD4 bajan a 200-350 células / mm3puede resultar en disfunción inmunitariaprolongada y un mayor riesgo para la TB persistente.

Early ART and IPT in HIV-Infected African Adults With High CD4 Count (TempranoTrial)Christine Danel1,et al.Temprano ANRS 12136 trial assessed the benefits of early ART and/or early 6-month isoniazid prophylaxis (IPT, 300mg/day) among HIV-infected adults with high CD4 counts and no WHO criteria for starting ART.Methods: Temprano was a randomized 2x2 factorial superiority trial conducted in 9 HIV care centers in Côte d'Ivoirefrom March 2008 through January 2015. Inclusion criteria were: HIV-1 infection, age >18 years, CD4 nadir 800/ul, andno criteria for starting ART according to the most recent WHO guidelines.Participants were randomized to one of four arms: ART initiation according to WHO criteria (WHOART); immediate6-month IPT plus ART initiation according to WHO criteria (WHOART-IPT); immediate ART initiation (EarlyART);immediate 6-month IPT plus immediate ART initiation (EarlyART-IPT).First-line ART consisted of tenofovir plus emtricitabine plus either efavirenz, zidovudine or lopinavir/ritonavir. Theprimary endpoint was severe HIV morbidity (AIDS-defining diseases, non-AIDS-defining malignancy, or non-AIDS-defining invasive bacterial diseases),or any-cause mortality at 30 months. The secondary endpoint was any othergrade 3-4 defining morbidity. We used multivariate Cox proportional models to compare outcomes between theWHOART and EarlyART arms, and between the IPT and no IPT arms. We tested for interaction between earlyART andIPT.Results: Of 2,076 patients randomized, 2,056 were included in the analysis (78% were women; 91% classified at WHOstage 1-2; median age 35 years; median CD4 nadir 465/ul; median HIV-1 viral load 4.7 log10 copies/ml). They werefollowed for 4,755 patient-years, during which 47 died, 175 experienced 204 episodes of severe morbidity (TB 85,invasive bacterial diseases 56, other AIDS-defining diseases 11, non-AIDS malignancy 5), 287 experienced 364episodes of severe grade 3-4 morbidity (hematologic 256, hepatic 31 renal, 22, cardiovascular 9, others 46), and 44(2.2%) were lost-to-follow up. There was no interaction between EarlyART and IPT. The risk of severe morbidity was44% lower with EarlyART vs. WHOART (Table) and 35% lower with IPT vs. no IPT. EarlyART significantly decreasedmorbidity overall and when restricted to patients with baseline CD4 >500/ul. The risk of Grade 3-4 morbidity did notdiffer between strategies.Conclusions: In Côte d'Ivoire, both immediate ART and IPT dramatically and independently decreased the risk ofsevere morbidity, and should be recommended as the standard of care for HIV.
WHO welcomes new evidence on the use of antiretrovirals forprevention and treatment of HIV infection
• Results from the TEMPRANO trial, conducted in Western Africa, have confirmed –for the first time in a randomized controlled trial – the individual health benefitsfor people who start antiretroviral therapy (ART) immediately after diagnosis. Thestudy demonstrated that ART can reduce the likelihood of severe illness by 44% inpeople starting treatment immediately, as compared to those starting ART onlywhen their CD4 levels drop to below 500/mm3 (or other levels recommended byWHO at the time they begin ART).
• Additionally, the study showed that isoniazid preventive treatment oftuberculosis (IPT) initiated among people living with HIV with a CD4 countgreater than 500/mm3 reduced the risk of severe illness by 35%, compared withthose not receiving such treatment. Early initiation of IPT did not increase the riskof resistance to isoniazid.
• The TEMPRANO results confirm WHO’s current recommendations for earlyinitiation of ART, and the use of IPT and co-trimoxazole (CTX) among peopleliving with HIV (PLHIV) to reduce morbidity. The results also represent importantadditional evidence to be considered by the guidelines panel for updating WHOrecommendations, regarding the optimal timing to initiate ART.
• The updated recommendations are planned for release in December 2015

-Subgroup analyses showed that effect estimates were higher (Pooled OR 2.28;95%, 1.52-3.04) for primary MDR-TB and moderate association betweenHIV/AIDS and MDR-TB among population based studies and no significantassociation in institution settings.
-CONCLUSIONS This study demonstrated that there is association between MDR-TB and HIV. Capacity for diagnosis of MDR-TB and initiating and scale up ofantiretroviral treatment, and collaborations between HIV and TB controlprograms need to be considered and strengthened

Ademas, hay muchos otros factores que puedeninluenciar el impacto de las intervenciones en cada entorno:diferencias en la farmaco-cinetica individual, genotipo de lascepas de BK, determinantes sociales…
Necesitamos algo nuevo para luchar contra TB-VIH…...pero con un uso mejor de las herramientas dx existentes,
un mas rapido dg de la TB, rapido acesso a los ARVs yexpansion de adecuado TPI,
y recordando los aspectos sociales de nuestros enfermos…… mucho podemos hacer.
