EL INTENSIFICADOR DE IMAGEN
Su función es:
Convertir el haz de rayos X que sale del paciente en una
imagen visible (luz visible).
Aumentar el brillo de esa imagen visible para que pueda

La relación beneficio/riesgo en el
intervencionismo es alta, pero el riesgo a altas
dosis existe y los efectos pueden ser
prolongados y severos.
Cuando las dosis de radiación son altas, los
pacientes no siempre son advertidos, ni
tampoco se les dice qué hacer cuando se
presentan síntomas o signos.

La situación (continuación):
Los profesionales médicos no siempre reconocen el daño por radiaciones cuando éste ocurre. A menudo sugieren diagnósticos equivocados sobre la verdadera causa del daño y algunos hasta niegan que la radiación podría causarlo.
La exposición ocupacional puede ser muy alta pero también puede reducirse, al mismo tiempo que se reduce la exposición innecesaria al paciente.

EFECTOS DETERMINISTAS Y ESTOCÁSTICOS
Efectos deterministas son aquellos que tienen un
umbral y su severidad aumenta con la dosis de
radiación p.ej. radiolesiones de piel. Para cada
tipo de efecto determinista existe un umbral
diferente: eritema, cataratas etc.
Se definen como efectos estocásticos aquellos
cuya probabilidad de padecerlos aumenta con la
dosis pero no su severidad, p.ej. la inducción de
cáncer. Se asume que no existe un umbral para
los efectos estocásticos.

EFECTOS EN LA PIEL
Las céls. más radiosensibles son aquellas que se encuentran en el sustrato basal de la epidermis.
Efectos:
– Eritema: 1 a 24 hs luego de unairradiación de 3-5 Gy
– Alopecía: 5 Gy es reversible; 20 Gy is irreversible.
– Pigmentación: Reversible, aparece 8 días post-irradiación.
– Descamación seca o húmeda: traduce hipoplasia epidérmica (dosis
20 Gy).
– Efectos tardíos: teleangiectasia, fibrosis dérmica.

LECCIONES APRENDIDAS DE PACIENTES CON DAÑO POR
RADIACIONES
Caso #1:
Procedimiento de ablación electrofisiológica.
Tres intentos en 4 meses, cada uno con más de 100 minutos de fluoroscopia.
Después del primer intento, se observa el eritema en el paciente, pero no es reconocido como consecuencia del procedimiento intervencionista.
Eritema en la espalda en proceso de curación.
Se requirió injerto en el brazo.

El calor invade lentamente desde afuera y debe afectar muchas células externas antes de alcanzar tejidos profundos.
Los rayos X causan un daño al azar en células muy localizadas internamente y no afectan células vecinas.
El calor es detectado antes que el daño ocurre.
Los síntomas del daño son prontos y progresan rápidamente y la extensión del daño se determina muy pronto.
La respuesta al tratamiento ya se manifiesta en algunos pocos días.

cristalino de una población vinculada a CI.
La elevada tasa de OSP observada en los CI (45%)

todos los elementos radioprotectivos que sea posible.
