Consequently, thereisaneedfor intervention trialstoexplorethe
issueof nutritional programming in humans. Such trials require
long-term follow-up, but in return they providean experimental
design, proof of causation and underpinning of practice. Human
intervention trialshavebeen used to explore the long-term
programming effects on brain development, bonehealth and risk
for cardiovascular disease10,11
Programming and Brain Development
Numerousstudieshave investigated the impact of early diet (e.g.
breast-feeding12,13) and malnutrition14,15 on later cognition.
However, thesestudies tended to beobservational and confounded
byother differencesbetween thegroups. Morerecently, randomized
trialsof early nutrition havebeen conducted. In theearly 1980s,
researchers investigated whether early nutrition, at a critical or
sensitivestagein thedevelopment of thebrain, couldhavelifetime
consequences in cognitive functioning for preterm infants.16 The
possibilitywasexploredbystudiescomparingcognitivefunctioning
in preterm infants fed enriched formula to thosefed standard
formula. Thefollowupshowedthat at 7.5to8yearsof age, preterm
infants fed standard formula demonstrated neurocognitive
impairment with a significant reduction in IQand a major 13-
point reduction in verbal IQin comparison to the infants fed
enriched formula11 (seeFigures1a, b). Unpublished data show
that theseeffects persist when tested in adolescenceand early
adulthood.
In addition, scientific toolssuch asmagnetic resonance
imaging nowmakeit possibletodemonstratethat specificaspects
of long-term brain structure are influenced by early dietary
insufficiency (unpublished data).
Programming and Bone Health
Alargecohort of prematureinfantswasrandomlyassignedtoearly
diet, and a variety of structural and functional boneindiceswere
studied in later childhood. Resultssuggest that infants fed a
standard formula in thefirst month had, by latechildhood, a
major increasein boneformation.17An increasein boneformation
and turnover isoneof the features of adult degenerative bone
disease, raising thehypothesis that optimal early nutrition is
important for reducing therisk of later degenerativebonedisease.
PROGRAMMINGOF CARDIOVASCULAR DISEASE
For thebrain and bones, meeting early nutritional needsand
promoting growth appear to havefavorableprogramming effects.
For programming therisk of cardiovascular disease, themajor
causeof mortality in theWest, different principlesappear toapply.
Cardiovascular risk maybeprogrammedboth pre- andpostnatally
F the latter isconsidered first.
Postnatal Programming
In distilling theevidence for nutritional programming of later
cardiovascular diseaserisk, thereappears to betwo important
influences: breast milk and postnatal growth.
Breast Milk and Later Cardiovascular Health
Breast-fedinfantshavebeen shown in observational studiestohave
lower risk of cardiovascular disease, hypercholesterolemia, obesity,
TypeII diabetesandhigh bloodpressure.18–21Thesedatacouldbe
confounded by socio-biological differences between breast-fed and
formula-fed groups. However, a causal relationship hasbeen
testable in preterm infants.22–25Here, it hasbeen possibleto take
infantswhosemothershad elected not to providebreast milk and
randomlyassign themtohuman milk fromamilk bank (donated
by lactating mothersunrelated to thesubject) or formula.22–25
In such trials, thosefedhuman milk, with aslittleas1month
(on average) intervention period, had lower blood pressure, LDL
cholesterol, ‘‘leptin resistance’’ (possible tendency to later obesity)
and insulin resistance.22–25 Theseeffects wereseen at 13 to 16
yearspost-intervention on prospective follow-up. Theeffect sizes
were large(seebelow), comparable to or greater than

Asfurther evidenceof causation, exploratory analysesof these
trialsshowedclear dose–responserelationshipsfor theseoutcomes:
thegreater thebreast milk intakein theneonatal period, thelower
therisk factor status in adolescence.
Thus, while theobservational data may still bechallenged as
inconclusive, thecombination of such evidencewith that from
formal intervention trials in preterm infants providescompelling
overall evidence that breast milk feeding reducesrisk of the
metabolic syndrome.
Impact of Rapid Postnatal Growth
Thetrialson preterm infants, cited above, compare infants fed
unsupplemented human milk versusenriched (preterm) formula,
andalso, in parallel, astandard formulaversusenriched formula.
Thesestudiesprovideexperimental evidence that dietspromoting
faster neonatal growth increaselater cardiovascular risk.22–25That
is, theinfantswhowerefed growth-promotingenriched dietshad
greater risk of developing key components of themetabolic
syndromeat follow-up.
Thisfindingwasnot just a featureof prematurity: infantsborn
full termbut small for gestation, whowererandomlyassignedtoa
growth-promoting formula, had higher diastolic blood pressure6
to 8 years later than thoseassigned to standard formula.10
Resultsof further analysesof thesestudieson pretermandterm
infantssuggested that it was indeed thegrowth acceleration that
explained theadverseeffectsof a nutrient-enriched diet on later
insulin resistanceand blood pressure.22,24
Postnatal Growth Acceleration Hypothesis
Thesefindings led to a proposed ‘‘postnatal growth acceleration
hypothesis’’ asa key to theearly programming of cardiovascular
disease.10.Consistent with this, later insulin resistancewasfound
greatest in infantsborn pretermwith acceleratedgrowth in thefirst
2weeks,24theperiodof fastest postnatal growth, andleast in those
who grewat a rate lower than that normally targeted (the
intrauterinegrowth rate). Such growth wasassociatedwith greater
endothelial dysfunction (reduced flow-mediated dilatation of the
brachial artery) in adolescence.26 Thus, early growth acceleration
programmed abnormal vascular biology associated with early
atherosclerosis, whereasslower early growth wasbeneficial
(Figure2).
Thesefindingsareconsistent with previousanimal data
showingthat ahigher planeof earlypostnatal nutrition andfaster
growth program propensity to themetabolic syndrome. Indeed,
adverse long-term health effectsof early growth acceleration
emergeasa fundamental biological phenomenon acrossanimal
species.27 Thus, growth acceleration during sensitive windows
reduces fat deposition in Atlantic salmon, lowersresistance to
starvation in speckled woodbutterflies, impairsglucosetolerance
and lifespan in rats, and reducescardiovascular diseaserisk in
non-human primates.
Furthermore, in humans, othershaveshown that childhood
growth acceleration isassociated with later insulin resistance,
obesity and CVD.28,29 However, growth acceleration isgreatest in
early infancy, suggesting that thisperiod may bemost critical.
Indeed, early growth acceleration isassociated with later obesity
anddyslipidemia raised concentrationsof insulin in infantssmall
for gestation. Thishypothesispredictsthat ahigh nutrient intake,
which promotesearly growth, would adversely program
cardiovascular health. Several observations, in addition tothetrials
cited above, support thisprediction. For instance, greater nutrient
intake in infancy isassociated with raised blood pressure in
adults.30
An important publichealth inferencefromthepostnatal growth
hypothesis is that theobserved advantagesof breast-feeding cited
abovemay bedueto theslower early growth of breast-milk-fed
infants.10
Fetal Programming
Prenatal programming has less immediate relevance to routine
clinical caresince it ismoredifficult to manipulatenutrition in
thefetus. Whileanimal studiesshowsensitivity toprogrammingin
fetal life,31,32 thishasbeen harder to approach in humans, with
fewrandomizedtrials, though work activelycontinuesin thisarea.
Most evidenceisretrospectiveandobservational andoften relieson
proxymeasuresfor earlynutrition, such assizeat birth. It isalso
unclear howtheobservedrelationshipsbetween small sizeat birth
and later cardiovascular diseaserisk9can betranslated into
practice recommendations. Moreover, thepublished evidenceon
cardiovascular risk suggests that the long-term effects of
postnatal nutritional intervention appear of substantially
greater magnitude than for, say, birth weight. Nevertheless, the
