Residual viral replication
It seems unlikely that viral replication occurs in plasma in
patients on ART
It seems very likely that viral replication occurs in
extraplasma sites (i.e. gut) in patients on ART
The need to intensify therapy for eradication purposes
still to be answered, but possibly a cure will not be
achievable without suppressing all residual viral

• Vorinostat (SAHA), a potent oral HDAC inhibidor, induces the expression of HIV from latently
infected cells ex vivo.
• Pilot, proof of concept clinical trial in 6 patients under antiretroviral treatment with plasma VL <50
copias RNA/mL for at least 6 months, > 300 CD4/mm3, and whose latently infected CD4 cells
had a positive response to SAHA in vitro (from 13 recruited patients).
• Determination of HIV intracellular RNA in resting CD4 obtained by leucopheresis in visit 2 and 5
and residual viremia by single copy assay.
• Vorinostat was safe and well tolerated. There were no adverse events of severity greater that
grade 1.

Reactivation of latent HIV with vorinostat:
A pilot clinical trial

• Pharmacokinetic studies were done at
visit 4 after a 400 mg dose of vorinostat.
• At visit 5, after 8 hours of a dose of 400
mg, a leucopheresis was performed and
intracellular HIV RNA was measured in
memory T cells to evaluate latency
reactivation: a mean 4,8-fold increase
was observed (range 1,5-10 fold
change).
• All the changes were statistically
significant without differences between
central memory and transitional cells.
• An increase in histone acetylation was observed.
• No effect on residual viremia
Reactivation of latent HIV with vorinostat:
A pilot clinical trial
• Se piensa que la participación de los linfocitos CD4 memoria-central (TCM) es clave
en el establecimiento del reservorio y la patogénesis del SIDA.
• Estudio RV254/SEARCH 010: tratamiento precoz con TAR durante infección aguda en Tailandia:
– 68 pacientes incluidos en estadios Fiebig I-III (media de tiempo de infección
15 días).
– Tratamiento en 2 días después de inclusión, seguimiento 2 años,
73% Subtipo CRF_01AE.
– Detección ultrasensible de DNA total e integrado en PBMCs (n=68),
biopsia de sigma (n=21) y subpoblaciones linfocitarias (TCM, TTM y TEM, n=9).
• Resultados: (1) muy poco DNA de VIH integrado en PBMCs (A) y sigma (B)
en pacientes en estadio Fiebig I (carga viral + y p24, ELISA y WB negativos).

• En parto test rápido anticuerpos VIH+ en la madre, WB +, CV: 2.423 cp/μL, CD4 644/μl, genotipado de
VIH: wild type y subtipo B.
• RN a las 30 horas: DNA VIH + VL: 19812 cp/μL (en dos muestras independientes).
• Se inicia tratamiento vía oral con AZT, 3TC y NVP a dosis terapéuticas, a los 7 días cambio de NVP
por LPV/r. No lactancia materna.

• Sólo detección marginal de VIH en 2 de 38 determinaciones en células
y 1 copia en plasma en SCA.
Problemas para la erradicación:
el reservorio latente es 50 veces mayor
• En pacientes en TAR, la estimación del reservorio de células latentemente infectadas con virus
inducibles en cultivo, era de 1 célula por cada 106 CD4 en reposo
(Siliciano et al 2003).
• Mediante técnicas de biología molecular (digital PCR) la frecuencia es mucho mayor: 300-
600/106.
¿Se explica esta diferencia por la detección de formas integradas defectivas?:
• Sólo en parte: mediante secuenciación de clones individuales de VIH integrado
(213 clones de 8 pacientes en TAR supresor), el 87,8 sería defectivo pero un 12,2% serían
provirus competentes:

• El inhibidor de diacetilasas de histonas Vorinostat/SAHA, ha demostrado in vitro
e in vivo que puede reactivar VIH latente sin inducir activación inmunológica,
y es por el momento la estrategia más desarrollada para purgar el reservorio
(Archin NM et al, Nature 2012).
• ¿Se reactivan todas las células latentemente
infectadas con Vorinostat?:
– Purificación de CD4 de 14 pacientes.
– Estimación de carga proviral por qPCR.
– Dilución límite y cultivo para estimar la
cantidad de VIH inducible conαCD3αCD28
(inducción máxima) y Vorinostat
(concentración terapéutica).
• La mayor parte de los provirus no son inducibles
con el estímulo deαCD3αCD28 (sólo posible in vitro).
• Una fracción todavía menor (0,13%) es activada
por Vorinostat se necesitarían estrategias mucho
más efectivas para la erradicación.
Problemas para la erradicación:

Prerequisite 1: – The HIV-1 reservoir was significantly reduced by the depletion of the T
cell pool by the conditioning regimen for stem cell transplantation
Prerequisite 2: – HIV-1 replication was abrogated in the remaining T cell fraction due to the
absence of activated CD4 T cells under immunosuppressive treatment
Prerequisite 3: – No new cell infection occurred since repopulating CD4 T cells were
resistant to CCR5-tropic HIV-1 infection because of the absence of CCR5 surface expression.

• Conclusión: la disminución del reservorio en estos pacientes puede ser debido a: (i) el efecto del cART
mantenido que impide la reinfección del injerto; (ii) a la quimioterapia que destruye los linfocitos del receptor;
(iii) a una reacción injerto contra huésped que destruye los linfocitos residuales infectados.
• Para evaluar la erradicación viral en estos pacientes es necesario: (i) detectar el RNA y/o DNA viral en tejidos,
especialmente en el GALT y (ii) suprimir el cART para confirmar la curación funcional o la erradicación.
