ACTUALIZACION EN GUIAS DE PROFILAXIS DE TRANSMISION MATERNONFANTIL

Caso clínico. Antecedentes. María tiene 17 años • Infección perinatal por VIH (C3).
• Encefalopatía asociada a la infección VIH. • Nadir CD4 <15%
• Problemas de adherencia y selección de mútiples resistencias al TAR. • ART previo: AZT, 3TC, d4T, NVP, NFV sin control virológico
• Incontinentia pigmenti. • Displasia ectodérmica ligada al cromosoma X (típicamente mortal en
varones)
Caso clínico. Antecedentes
•Maria nació en 1996 y fue adoptada a la edad de 3 meses.
•Valorada por primera vez en Madrid en 2006:
• Lesiones hiperpigmentadas en patrón lineal y paraparesia espástica leve.
• CD4 582 células/ mm3(23%), carga viral VIH-1: 5.1 log • Resistencia genotípica: L210W, T215Y, G190S y D30N.
• Cambio a ddI + ABC + EFV + LPV/r

• En julio de 2013 (17 años de edad), acude a consulta por retraso en la menstruación. • Se confirma un embarazo (8 semanas)
•Una nueva carga viral en plasma muestra 22.608
copias / ml (4.35 log) y una nueva caída en CD4 hasta 370 células/mm3
• Estudio de resistencias genotípicas e interpretación:
resistencias a ITIAN, ITINN y algunas mutaciones menores a IP

• Cambio TAR a DRV/r bid + RAL + TDF/FTC • Evaluación tropismo CCR5 (que muestra R a maraviroc)
• Sexo fetal en sangre materna: XY.
• María y su pareja decidieron continuar con el embarazo y someterse a una amniocentesis a las 20 semanas de gestación (CV <20 cp / ml ), para realizar un aborto terapéutico en caso de que el feto varón tuviera el gen IKBKG, asociado a la incontinencia pigmenti.
• Se refuerza adherencia: • Psicólogo • DOT • Ingreso a las 16 semanas de gestación
Evolución • TAR posteriormente en gestación con buena
adherencia.
•Parto vaginal. RNAT con PAEG
• Profilaxis con AZT oral durante 4 semanas
• Lactancia artificial exclusiva
• Se descartó infección VIH-1 en el hijo

Raltegravir en gestación: 2001-2015 • 278 maternal–infant pairs
• Maternal outcomes: • The only consistently reported maternal AE was a reversible and transient
increase in liver transaminase
• Infant outcomes: • HIV-infected children: 2 cases (0.7%) • 1 report of an intrauterine fetal demise at 32- week gestation associated with
pregnancy-induced maternal hypertension • SGA: 14% in a multicenter trial • 1 infant who had reduced RAL elimination [low UGT 1A1 activity] required
phototherapy for the treatment of hyperbilirubinemia.

Alerta sobre posible teratogenicidad del DTG
Reciente estudio en Botswana (n = 11.558) ha mostrado que el 0,9% de los RN, cuyas madres estaban en
tratamiento con DTG en el momento de la concepción han presentado defectos del tubo neural, frente al
0,1% de los bebes, hijos de madres bajo pautas libres de DTG.
RECOMENDACIONES DE LA EMA RESPECTO AL DTG:
Conclusiones
• En los últimos años existe un interés creciente en el estudio de los inhibidores de integrasa (raltegravir y dolutegravir) en la gestación.
• Raltegravir presenta una tolerabilidad y un perfil de seguridad adecuado para su uso en la gestación, en base a los estudios publicados. Por sus características , RTG pueden tener un papel importante en situaciones de alto riesgo de transmisión especialmente en aquellas madres con carga detectable cercana al parto o diagnostico tardío.
• En el caso de dolutegravir los datos son más limitados. La alerta relativa al estudio de Botswana ha conllevado a la recomendación de evitar la exposición a DTG en mujeres con deseo de gestacion y por tanto también en el primer trimestre , al menos hasta que se tengan mas datos des seguridad.
