Caso Clínico
Paciente de 53 años, menopáusica, diagnosticada en Marzo-2012 de cáncer de
mama derecha pT2 pN0 M0 con RE-, RP-, HER2 -, Ki 67 40%. Recibe QT
adyuvante (FACx4 Paclitaxelx8) y RT loco-regional.
En Dic-13 (ILE 15 meses), acude por astenia y tumefacción EID. En las pruebas
complementarias se objetiva CA 15.3 140 U/ml, metástasis hepáticas y trombosis
venosa profunda. ¿Qué tratamiento recomendaría?

Nab-paclitaxel (AbraxaneÒ) is an albumin-bound form of paclitaxel that utilizes the natural
properties of albumin to improve paclitaxel delivery to the tumor. It is licensed for use in
metastatic breast cancer (MBC) at a dose of 260 mg/m2 Q3W based on its superior therapeutic
index versus conventional paclitaxel 175 mg/m2 Q3W demonstrated in a Phase III study. In a
post-hoc analysis, nab-paclitaxel treatment was associated with rapid and dramatic tumor
responses in patients with poor prognostic factors (visceral dominant disease, ‡3 metastatic
lesions), suggesting it may be a preferred treatment for these patients. Moreover, significant
efficacy has been seen with nab-paclitaxel 100 and 150 mg/m2 QW 3/4, suggesting it may be
possible to tailor use of this agent in the future.

Nab-paclitaxel (AbraxaneÒ; Celgene Corpora-
tion, Summit, NJ, USA) is a next-generation
taxane licensed in Europe, the USA and
many other countries around the world for the
treatment of metastaticbreast cancer (MBC) [1,2].
Developed using nabTM
technology, it comprises
albumin-bound nanoparticles of paclitaxel that
arerapidly released into thecirculation following
intravenous (i.v.) administration [3]. Nab-
paclitaxel is also devoid of solvents, eliminating
the need for immunosuppressive premedications
and improving the tolerability profile of this
agent compared with traditional taxanes[4].
Nab-paclitaxel is formulated with human
serum albumin which, in addition to its role as
an endogenoustransporter of hydrophobic mol-
ecules, specifically binds to albumin receptors
such as gp60 on the endothelial cell surface,
activating caveolin-1 and triggering the forma-
tion of vesicles to initiate endothelial transcyto-
sis [5]. It is hypothesized that these mechanisms
facilitate the preferential delivery and accumula-
tion of paclitaxel at the site of the tumor [6,7].
Indeed, variouspreclinical studieshaveprovided
evidence to support the suggested active trans-
port mechanisms of nab-paclitaxel [7,8], with
early Phase I and pharmacokinetic (PK) studies
indicating that this agent also has a distinct PK
profile, including a greater mean fraction of
unbound paclitaxel with nab-paclitaxel versus
conventional paclitaxel (6.3 vs2.4%, p < 0.001)
[9] and linear PK across the clinically relevant
doserange(i.e., 80-300 mg/m2) [10,11].
Given the differences in composition and
mechanism of action of nab-paclitaxel com-
pared with traditional taxanes, it is not surpris-
ing that thisagent hasdemonstrated impressive
efficacy with an acceptable tolerability profile in
MBC aspart of aPhaseIII study [12], with addi-
tional Phase I and II studies suggesting that it
can also be administered at different dosing
schedulesand aspart of combination therapy in
this setting [4]. However, despite these benefits,
little is known regarding the optimum use of
this agent in each individual patient as part of
our routine clinical practice. Here, we review
the clinical data for nab-paclitaxel in MBC and
discuss why the unique properties of this agent
might make it a preferred treatment option for
many patientswith MBC, including those with
aggressive disease who require fast-acting and
effectivechemotherapy.
