•Identificar variables predictivas de respuesta al tratamiento.

Exposición del caso:Paciente de 48 años, enviado de la consulta de H t l í J i d 1998 l ió Hematología en Junio de 1998, para evaluación diagnóstica y terapéutica de infección crónica por VHC.
AP: -Enfermedad de Von Willebrant (año 1974).( )-Recibió transfusiones múltiples (años 1974-1989).-Hepatitis aguda en el año 1980, de evolución
l d t i d t i l d prolongada y manteniendo transaminasas elevadas desde esa época.-No otras enfermedades asociadas-No otras enfermedades asociadas.-No hábitos tóxicos.
Clínicamente asintomático, con exploración física normal.

Modificaciones de dosis:
Se disminuye ribavirina a 800 mg/día en el 3º mes de tratamiento por anemia y descenso de peso corporal tratamiento, por anemia y descenso de peso corporal a 72,4 Kg.
Tratamientos concomitantes:
Paracetamol a demanda, que tomó ocasionalmente.
Tiroxina (Dexnon®) 100mg/día, que se pautó al finalizar el tratamiento y se mantuvo durante 7 meses

Evolución post-tratamiento:
El paciente se mantiene en seguimiento tras la finalización del tratamiento antivírico, con evaluaciones periódicas de sus parámetros bioquímicossus parámetros bioquímicos.
Se realiza elastografía en Diciembre 2007: 10.1 Kpa (F3-F4, Se realiza elastografía en Diciembre 2007: 10.1 Kpa (F3 F4, fibrosis significativa)
Se repite nuevamente en Diciembre 2009: 14 Kpa(F4, cirrosis)
Se procede entonces a una nueva valoración terapéutica.

CONCLUSIONES1.‐ El tratamiento de la hepatopatía por VHC requiere un abordaje terapéutico individualizadoun abordaje terapéutico individualizado.
2 Diferentes biomarcadores predictivos (bioquímicos2.‐ Diferentes biomarcadores predictivos (bioquímicos, genéticos, histológicos…) han sido evaluados sobre la probabilidad de alcanzar una respuesta virológica sostenidaprobabilidad de alcanzar una respuesta virológica sostenida.
